Email teachers (reminder about the last email from week 4) - If student is receiving a D or F: - Talk to the teacher and document it on SPED FORMS - Call Parent (document on SPED FORMS) - If necessary: Weekly progress report (attached) See students or send them a copy of the community service form so that they may think about it and investigate where they will be doing their community service hours * Weekly progress report * Community Service form

Email teachers that you will be coming to their classrooms to observe the STUDENT and the student behavior and overall performance in their class- Let teachers know that you will be looking at what specific behaviors the student has in their classroom and what type of strategies the students are using to perform better academically Also make a copy of them or email them some of the strategies that are good to use with the student (If EL check SDAIE strategies for Academic Achievement) * Strategies for students with Specific Learning Disabilities * SDAIE strategies for Academic Achievement

Contact English teachers and get a writing sample for your student (essay, short story, etc.) -opt: Have student write a reflection on their writing * Writers Reflection on finished paper

Classroom Observations (student focused) -Go to the students class FOR INCLUSION IF APPLICABLE: take the chart that you filled out from week 3 to verify those modifications you suggested are working towards the individual needs of the student -Verify that the student is receiving accommodations or modifications - Behavior classroom observations: Best to use SYHS SpED behavioral observation Form * SYHS SpED Behavioral Observation * Class accommodations at a glance * accommodation checklist * Daily progress monitoring * Intervention Log * Behavior consequences Log * Behavior Observation Checklist Special Note: If a maladaptive behavior is observed either create a behavior goal or a Behavior Support Plan (BSP) on SPEDFORMS. Progress Reports: Access chancery and get a copy of the student grades, print it and file it in their binders -IF student has D or F in a class where they had a C or better during the fist progress report: highlight it and contact parent/teacher to make an intervention plan and schedule a meeting if necessary If student CONTINUES with a D or F in a class where they already had a D or F the 1st progress report: then schedule a parent meeting with the teacher (use the action plan template at the meeting) - When you contact parent/teacher make sure you
