This agreement was made by and between: National College of Science and Technology, through its Discipline Officer Rev. Ptr. Ferdinand R. Villanueva Sr. MDiv And I, Krytan Yrielle P. Billiones student of BACHELOR OF SCIENCE IN ACCOUNTANCY Department, agreed to the following conditions; After having been given the right to be heard and after having been found to have violated section 10 NO SMOKING POLICY of the NCSTs student handbook and RA 9211, I understand that my acts constituted a major offense which sanctions me with (1) year suspension of Good Moral Certificate and a five (5) days or an equivalent 40 hours of community service. However, I promise not to commit any other violation under the student handbook and promise to abide by the provisions therein. I understand that a breach of this agreement would mean a heavier penalty. I am executing this agreement this 11 November 2012 at the National College of Science and Technology, Amafel Bldg. Aguinaldo Highway, Dasmarias, Cavite.

Note: Good Moral Certificate shall only be issued upon completion of the abovementioned penalty.
