AFFIDAVIT OF EMPLOYMENT I, CHESSA GUNDA ZALETA, of legal age, Filipino citizen, single, and a resident of Dulong-bayan Ilaod, Brgy. I, Daet, Camarines Norte, after having been duly sworn to in accordance with law, hereby depose and say: That I am a dancer by profession; That I have been employed as a DANCER at KUDOS GRILL BAR & RESTAURANT owned and operated by CN KARAKANAN INCORPORATED, from the period of December 5, 2005 up to the present; That I perform from 9:00 p.m. to 1:00 a.m. with three sets of shows at exactly 9:00 p.m., 11:00 p.m., and 1:00 a.m., twenty minutes per show, and that the total duration is 60 minutes daily from Monday up to Friday; That attached herewith are the following documents of the above-named business, to wit: 1. Certificate of Business Name Registration from the Department of Trade and Industry 2. Certificate of Incorporation issued by the Securities and Exchange Commission That I am executing this affidavit to attest to the truthfulness of all the foregoing facts, and for whatever legal purpose this may serve.
