DEVELOPING A CONCEPT MAP Step 1: Develop a Basic Skeleton Diagram State what your patients key problems are

based on data you can collect. Key problems = concepts Problem #1 Problem #2 MEDICAL DIAGNOSIS Problem #4 Misc. Problem #3

Step 2:

State medical Diagnosis Think about the problems this diagnosis has created for the patient based on the assessment data Place these problems around the medical diagnosis Research the medical diagnosis, include meds, Rxs, labs, diet (how is it all related to the medical diagnosis?) Analyze and Categorize Data Problems that extend out from the medical diagnosis will become nursing diagnosis Review physical assessment data if data fits in more than one box, place it wherever it needs to go Explain why you put each item in each box. If you cannot, perhaps you have not collected enough data Once patient data is analyzed and placed, add key assessments to the central box. What is the most important problem to watch out for? Utilize the Misc box for items you dont know where to categorize Step 3: Label Diagnoses Analyze Relationship between Problems Assign appropriate nursing diagnosis to the listed problems Ex.: BP problems (Ineffective Tissue Perfusion Nutrition / Imbalance Nutrition: Less than body requirements Nursing diagnoses are focused on the patients responses to a health problem. In a concept map, the focus is on actual problem, not potential problems
