Mikaylas Behavior Plan (Revised on 05/02/12):

Ms. Lee (classroom teacher) will provide Mikayla with the CICO sheet each morning. If Ms. Lee is absent, she will leave a CICO sheet and notes about Mikayla in the sub plans. If it is last minute, she will email Ms. Owen to let her know and Ms. Owen will talk to the sub about Mikaylas plan and provide Mikayla with a CICO sheet. Ms. Owen (or Mr. McNulty/Ms. Lee when she is busy) will be the CICO coach that will see her in the morning, after lunch and at the end of the day. Currently, Mikayla has to be escorted wherever she goes by a teacher or another student. The team will look into this once Mikayla goes a while without any stealing incidents. Mikaylas bus has assigned seats due to the behavior problems between Mikayla and two other students.

CICO Sheet Specifics


I was truthful and did not steal. I was respectful. I acted appropriately in class.

In this section, circle L if Mikayla lied, S if Mikayla stole something, or a 2 if she did neither of those things. This section is about being respectful to teachers and other students. If Mikayla has an attitude, rolls her eyes, etc. give points accordingly. This section is for on-task behavior, participating, etc.

Initial ND/R

Mikayla will sometimes not earn points in your class because she has lied/stolen something at another time. Do not tally the points but rather initial with a pen, and write R if she got a referral.

When she makes her goal, Mikayla will receive a reward from Ms. Owen when she checks out. If Mr. McNulty or Ms. Lee check Mikayla out when Ms. Owen is gone, they can her a reward or wait for Ms. Owen to give Mikayla one the next day. If they do, they will write REWARD on the sheet so Ms. Owen knows Mikayla already got a reward. After several consecutive days of making her goal, Mikayla can earn a bigger reward (such as a lunch party, nail party, art activity, etc).

Kyles Behavior Plan (Revised on 10/01/2010):


Kyles daily behavior chart has been sent in PDF format and paper copies to both his classroom teacher and social worked. Every morning, Kyle will be given a chart by his classroom teacher that will stay at his desk. It should be premarked with the date, goal score and corresponding goal score line on his thermometer (as shown). At the end of each class, the classroom teacher or case manager will give circle Kyles points for staying on task, taking his time doing work and being organized. They will then add up the total for that class and write it in the line. At this point, Kyle can fill in the appropriate amount of boxes on his thermometer. Note: As shown on the example to the right, each box counts for a point on the thermometer. In the example, he received 5pts and colored in the corresponding amount of boxes. At the end of the day, the classroom teacher will check in with Kyle to see if he made his goal for the day. Kyle will know this because his Goal Score is written at the top of the chart and the corresponding line will be marked on his thermometer at the beginning of the day. Either the teacher or Kyle will write the total amount of points he earned that day at the top of his chart. If Kyle did meet his goal, he will receive the appropriate reinforcement.
