AFFIDAVIT OF LOSS

Mr. Jonathan V. Villabroza, a 2nd year student taking up Bachelor of Secondary Education Major in English with Student Number 2011-00306-TG-0, does hereby depose and say: That sometime on March 15, 2013 I lost my 2nd Semester Registration Card for the first time; when I was already at home, I discovered that my registration card was no longer in my possession, that earnest and diligent effort to locate the same was rendered futile; and at the same time was not confiscated by authorities for violation if any law or used for illegal purposes. In view thereof, I am requesting that a duplicate copy of my registration card be issued in my favor. I undertake to be more careful and prudent next time so that this incident will not be repeated. I am willing to submit myself to a corresponding disciplinary action should I lose the said document again.

(March 26, 27 and April 1) Ready your regi with fully paid*, ID, certificate of OL Evaluation for Faculty 2nd sem, strip of paper during the MS election 2013 and settle your liabilities especially ung di nkasama s team building. -kung may nawawala kayo s mga nabanggit, ready your *letter with VALID reason with signature of your parent/guardian and ID nung pipirma sa letter niyo. -so far, eto p lang ang naalala kong kailangan but wait for the further announcements March 26, 27 and April 1: Maghihintay lang po kmi s school s org rum ng 10AM to 5PM kung di nyo kmi maabutan, pasensyahan n lang. P.S. di po MSO ang maghahabol sa inyo but instead dis tym KAYO ang maghahabol sa amin, kaya please walang mgpapa VIP. Dalhin ang mga dapat dalhin at asikasuhin ang dapat asikasuhin.
