Vision
Healthy Aging Through Community Empowerment

Mission
To engage with healthcare providers, community-based organizations, businesses, and policy makers in order to promote healthy aging through preventative health programs, reducing disparities in education and access, and creation of healthy community initiatives for sustainable healthy aging.

Goals
1. Establish relationships in collaboration with community partners towards widening access to services 2. Establish a data analysis unit to collect, track and analyze data which is appropriately reflective of senior health and well-being 3. Promote healthy lifestyles and healthy community initiatives 4. Provide health education through educational initiatives, and the organization and facilitation of an annual Healthy Aging Conference.

Goals with Initiatives and Deliverable Dates 1. Establish relationships in collaboration with community partners towards widening access to services a) Contact SBC Department of Public Health and SBC Behavioral Health to establish working relationship and partnership in the sharing of data regarding the elderly by December 19, 2012. b) Contact SBC Board of Supervisors (BOS) to establish the opportunity for a presentation of the vision, mission, goals and initiatives of the Healthy Aging Committee in order to gain their support, acknowledgement and participation by February 22, 2013. c) Contact local universities to establish relationships in the collection of data on the elderly, partner research opportunities and participation in the Healthy Aging Conference by February 5, 2013. d) Contact State and Federal officials to establish information-sharing relationships and support and participation for the Healthy Aging Committee and Conference by March 15, 2013. e) Encourage the development of more medico-social partnerships for elderly health promotion, bringing together faith-based organizations, senior centers and the business community by March 2013. f) Enhance patient self-efficacy with regards to quality and cost of care through partnerships and collaborations between the medical community and communitybased service programs, including evidence-based programs by August, 2013. g) Encourage senior housing areas (buildings and naturally occurring retirement communities) to consider medical and social programming to support aging in place. On-going activities with DAAS/SAC Committees. h) Identify and establish home care services, including use of new technology, to help the elderly remain and prosper in their homes by July 2013. i) Establish a Healthy Aging Collaboration Model of all resources pertaining to services and professionals as the model of choice for the Inland Empire elderly by July 2013.

j) Send out Save the Date cards/flyers to all entities above for the Healthy Aging Conference by February 4, 2013.

2. Establish a data analysis unit to collect, track and analyze data which is appropriately reflective of senior health and well-being a) Establish a data research and analysis unit that can provide and maintain the collection and retention of appropriate measures by October 17, 2012. b) Identify appropriate objectives measures of senior health and wellness by January 15, 2013 c) Establish a data repository of providers, community-based organizations, faith-based organizations and businesses by March 30th, 2013. d) Establish a database of statistics on the elderly, their trends, and survey results of unmet needs by August 2013. e) Develop a project plan to seek out grant opportunities for promotion of the vision, mission, goals and initiatives of the Senior Affairs Commissions Healthy Aging Committee by June 2013 f) Prioritize health issues and develop a project plan to effectively respond to emerging needs and trend lines by June 2013 3. Promote healthy lifestyles and healthy city initiatives a) Promote, and with the help of community and business partners, establish nutrition support, transportation support, access support and social support programs for the elderly. On-going activities with DAAS/SAC Committees. b) Work with the SAC Nutrition Committee to address the issues of nutrition and health in the elderly, ensuring more data is presented in annual nutrition publications and helping to support and grow nutritional programs by December 2013. c) Work with the SAC Access and Transportation Committee to address the issues of better access and safety for the elderly by providing collected data on the development of homes and facilities with regard to the ADA compliance regulations by December 2013. d) Expand social support programs for the elderly by providing information and access to medical and social programs which support aging in place by December 2013. e) Develop a database mapping a network of dentists, audiologists, ophthalmologists, nutritionists, and other practitioners who will provide services to the lowest income seniors in our community by February 2014. f) Encourage community emergency planning to take into account the vulnerabilities and capacities of older people by February 2014. 4. d) Develop Healthy Aging Conference Program for the two-day event by February 9, 2013. e) Secure keynote speakers for the Healthy Aging Conference by February 16, 2013. f) Send out Healthy Aging Newsletter to the community, all agencies, and government organizations regarding plans for the Healthy Aging Conference by February 20, 2013. g) Develop a Healthy Aging Conference Timeline worksheet for the committee by January 30, 2013. h) Secure participation from SBC Departments of Public Health and Behavioral Health, and Supervisors of the county for the Healthy Aging Conference by February 1, 2012. i) Deliver presentation to the SBC Board of Supervisors on the DAAS/SAC/Healthy Aging Committee and Conference to gain their support and participation by February 26, 2013. Secure City, County, State and Federal officials commitments of participation in the Healthy Aging Conference by March 20, 2013.
