I, WOODY P. DUMDUM, of legal age, Filipino, married, and resident of FVR Village, Fatime, General Santos City, after having been duly sworn to in accordance with law, do hereby depose and say: 1. That I am the father of JOHN RHEIN ESTRADA; 2. That he was born last June 23, 2006 at General Santos City, Philippines; 3. That the birth was recorded under Registry No. 2006-7264 on July 3, 2006; 4. That JOHN RHEIN ESTRADA, my son was not able to use my surname for the reason that when he was born, me and his mother ARCELE B. ESTRADA are not married yet; 5. That I acknowledge and support my son and allow the use of my surname; 6. That I hereby certify that the statements made herein are true to the best of my knowledge and belief; 7.
