no hives, no rashes, no drying of skin, no change in color, no pruritus, no suspicious masses or lesions, no changes in nails and hair, no profuse sweating HEAD: With persistent headache that is severe (pain scale of 7/10) and sudden in onset, no history of head trauma or seizures EYES: No lesions or redness, no unusual lacrimation or discharge, no frequent rubbing, no evident disturbance in vision, no crossing or tendency to cross, no puffiness and redness, no injuries EARS: No evident hearing impairment, no excessive ear drainage, no ear pain, no signs of ear infection, no tinnitus, no vertigo NOSE: No watery or purulent discharge, no epistaxis, no itching MOUTH/THROAT: With sore throat and difficulty in swallowing, no bleeding gums, no mouth ulcers or any lesions, no hoarseness, no dental defects NECK: No noticeable swelling of lymph nodes or any masses CARDIOVASCULAR: No known heart disease, no fainting spells, no bluish discoloration of lips and nails, no edema GASTROINTESTINAL: With regular bowel habit (1 x a day every morning), no vomiting, no jaundice, no evidence of abdominal pain or distention, no rectal bleeding GENITOURINARY: No hematuria, no urgency, no frequency, no poulyuria, no oliguria, no nocturia, no dysuria, no incontinence, no enuresis, no redness, no sores or any other lesions, no abnormal urethral discharge, not yet menstruating MUSCULOSKELETAL: No weakness and stiffness on extremities, no bone, muscle and joint deformities and swelling, no limping HEMATOLOGIC: Not anemic, no easy bruising or bleeding, no history of bood transfusions ENDOCRINE: No goitrous enlargement, no heat or cold intolerance, no polyuria, no polyphagia, no polydipsia, no growth problems NEUROLOGICAL/BEHAVIORAL: Not irritable, no fainting episodes, no

seizures, no tremors, no weakness or paralysis, no sleep problems, no incoordination, no behavioral changes
