I am requesting fundin g for justice programs in fi scal year 20 I O. The entity to receive
funding for this project is the Chicago School of Professional Psychology, located at 325
North Wells Street, Chicago, Illinois, 60654. The funding would be used to provide
3,200 contact hours of behavioral health services to underserved residents in the South
Side of Chicago . I certify that neither I nor my spouse has any financial interest in this
