Volunteer Permission Slip/ Emergency Contact Form for Minors

All volunteers under the age of 18 who wish to volunteer at Cranbrook Institute of Science must have a permission slip on file with their parent/guardians signature and emergency contact information. Hand in the completed form at check-in on the first day of volunteering. Without a completed and signed form, those under age 18 will not be eligible to volunteer.
