11) SHIP OWNERS WILL ONLY COMMENCE PAYING CREW SALARY ONLY, WHEN THEY ARE

D) EARLY REPATRIATION DUE TO INJURED / SICKNESS ( DOCTOR DIAGNOSIS UNFIT FOR WORK OR DUTY ONBOARD OR AT OWNERS DISCRETION ) BUT MUST BE AT LEAST 6 MONTHS ONBOARD VESSEL ( RETURN AIR-TICKET ). 13) IN THE EVENT CREW DID NOT COMPLETED THE CONTRACT DUE TO DISPLICINE MATTERS/TERMINATION OF EARLY CONTRACT/ BREACH OF CONTRACT/ ETCS, THEN ALL COST INCURRED FOR SIGNING ON/ OFF FROM VESSEL WILL BE FOR HIS PERSONNEL ACCOUNTS. (CREWS) 14) CREW WHO ARE FOUND TO BE SELF-PROMOTING THEMSELVE OR OFFER THEIR DIRECT SERVICE TO THE SHIP OWNERS CONSIDERED AS BREACH OF CONTRACT. 15 ) COMPENSATION: 15.1 SICKNESS AND INJURY : A) CREW IS ENTITLE TO NORMAL MEDICAL TREATMENT AT SHIP OWNERS EXPENSES ACCORDINGLY TO "P&I" CLUB RULES & REGULATIONS, BUT EXPENSES ARISING FROM THE FOLLOWING CAUSES. SHALL BE BORNE BY THE CREW MEMBER'S CONCERNED ITSELF:i ) SEXUALLY TRANSMITTED DISEASES ii) OPTICAL TREATMENT iii) DENTAL TREAIMENT OTHER THAN EXTRACTION OF TEETH iv) FALSE DENTURES AND SPECTACLES v) INJURY SUSTAINED OUT OF PERVIOUS EMPLOYMENT B) CREW WHO GOES ON SICK LEAVE RECOMMENDED BY A DOCTOR APPROVED BY SHIP OWNER'S WILL RECEIVE FULL WAGE (BASIC WAGE ONLY) DURING THE MEDICAL PERIOD BUT SUBJECT TO A PERIOD NOT EXCEEDING 2 MONTHS. COST OF MEDICATION CONSULTATION SHALL BE BORNE BY SHIP OWNER'S UP TO A MAXIMUM OF 3 MONTHS. 15.2) COMPENSATION FOR DISABILITY AND DEATH: A) CREW IN THE EVENT OF 100% DISABILITY OR DEATH CAUSED DUE TO WORK RELATED DURING HIS CONTRACT PERIOD WILL BE ENTITLED TO A COMPENSATION OF USD 25,000. B) INCASE OF PERMANENT PARTIAL DISABILITY. THE AMOUNT OF COMPENSATION WILL BE CALCULATED ACCORDINGLY TO THE FOLLOWING TABLE AND APPROVED BY SHIP OWNER'S PHYSICIAN (USD 25,000 x PERCENTAGE 40% 35% 30% 35% = AMOUNT OF COMPENSATION) 01) ONE ARM FROM SHOULDER DOWN 02) ONE ARM FROM ELBOW DOWN 03) ONE HAND 04) ONE LEG FROM HIP DOWN

C) THE PROPERTY AND UNPAID WAGES OF DECEASED CREW SHALL BE GIVEN / PAID SHIP OWNER'S THROUGH PRINCIPAL TO HIS BENEFICIARY. 15.3) COMPENSATION FOR LOSS OF PERSONNAL EFFECTS: CLAIMS FOR LOSS OF PERSONNAL EFFECTS AND COMPENSATION IN THE EVENT OF LOSS DUE TO ACCIDENT, FIRE OR OTHER MISHAPS AFFECTING THE SHIP.THE CREW SHALL BE COMPENSATED UP TO A MAXIMUM AMOUNT OF USD 1,000 ACCORDINGLY TO THE ACTUAL AMOUNT OF THE LOSS OR DAMAGE OBJECTS BUT THERE WILL BE NO COVERAGEFOR CASH AND CIGARETTES. 16) YOU ( CREW ) SHALL KEEP THE TERMS AND CONDITION HEREIN CONFIDENTIAL IN PERPETUITY SUCH INFORMATION SHALL NOT BE RELEASED TO ANY EXTERNAL PARTY WITHOUT OUR CONSENT. (GUILTY OF THIS IS DEEM AS BREACH OF CONTRACT) 17) THE CREW ACCEPTED TO DEDUCT USD1500 FROM SALARY AS GURANTEE BACK WHEN MONEY(DEDUCT FROM 3 MONTHS , USD500 PER MONTH) WILL RETURN COMPLETE CONTRACT. 18) THE CREW ACCEPTED TO DEDUCT RETURN AIR TICKET FARE FROM GURANTEE MONEY, IF THE CREW NOT COMPLETE CONTRACT. 19) THE CREW ACCEPTED ABOVE STIPULATED TERMS & CONDITIONS.
