LPN EVENING REQUIREMENTS

PLEASE READ THE FOLLOWING INFORMATION WHEN YOU APPLY

TO THE EVENING LICENSED PRACTICAL NURSING PROGRAM

1. You must take the TABE Test or provide proof of an A.A. or College degree from a
regionally accredited institution .There is a $20.00 application fee.
2. Provide either a copy of your high school diploma from an accredited high school or
GED. Please check website at www.accreditedschools.org to determine acceptable
accreditation
3. Successful completion of a FBON ( Florida Board Of Nursing ) approved Articulated
Nursing course (165 hours).It must be completed and documentation provided prior
to acceptance.
If you do not have this C.N.A. you may apply to take it at LWIT.

Please note that enrolling and successfully completing the Articulated Nursing course DOES
NOT GUARANTEE ADMISSION TO THE NEXT LPN DAY AND/OR EVENING CLASS,IT
SIMPLY MAKES THE CANDIDATE ELIGIBLE FOR ADMISSION.
ADMISSION AND ENROLLMENT DEPEND ON AVAILABILITY ONLY.

Other requirements that will be needed on or after you are informed by mail that you are
accepted into the program

a. Background check (additional cost-approx.$60.00 )

b. CPR-BLS for Health Care Providers ( additional cost –approx. $45.00)
c. Physical Exam ( additional cost)
