RE: Revised Petition Form to Take the Bar Examinations

Quoted hereunder, for your information, is a resolution of this Court dated MAR 21 2006.
"Bar Matter No. 1559.- Re: Revised Petition Form to Take the Bar Examinations.- The Court Resolved to APPROVE the
revised form of the Petition to Take the Bar Examinations, incorporating therein the requirement that the candidate shall
submit a certification by the law dean that the candidate has no derogatory record in school and, if any, the details and
status thereof, thus:

COMES NOW the undersigned petitioner and to this Honorable Court respectfully states:
