Occupational accident 130-596 Fall overboard from smaller tank ship

Summary:

During a bunker operation in Hirtshals Harbour on 18 January 2012, the skipper of a small tank ship fell overboard and drowned. The accident occurred at 1800, while the tank ship was departing from a fishing vessel that had been bunkered. After having loosened the stern fast on board the fishing vessel, the skipper entered the tank ship where he placed some papers in the wheelhouse. He had left his mobile phone and a glove on board the fishing vessel at the filler plug. It is likely that, in an attempt to get hold of these effects, he fell overboard.

Safety recommendations:

The Danish Maritime Accident Investigation Board recommends that the owner and the skippers consider how precautions can be made in the future to increase the safety of the work, reflecting the conditions of every single work process. In this connection, attention is drawn to the possibility of involving a third party to take account of the difficulty that individuals familiar with the daily work routines may have recognizing any new risks.
