Injury and Liability Waiver Assumption of Risk and Agreement to Participate

In consideration of being permitted by ALSP to the following waiver and agreement:

NCR to participate in the Conflicts of Law, I agree

I understand the risk in joining competitive sports in the 2012 DLSU Conflicts of Law. I realize that those risks include falls, injuries, and etc. while participating or observing the activities during the event. I am physically fit and know no medical or health reason why I should not participate in the activities that take place in the Conflicts of Law. I hereby voluntary release De La Salle University Manila, De La Salle University Law Student Government, and the Association of Law Students of the Philippines, from any and all liability claims, demands, or actions, which are related to, arise out of or are in anyway connected with my participation in this activity. My signature below indicates that I have read this waiver form and that I understand its stipulations. I agree to be bound by its terms and conditions.
