Report of Rating of Eligibility authenticated by the issuing authority E. Two (2) pieces 2x2 colored picture with nametag, one (1) 4R half body picture and one (1) 4R whole body picture; F. Transcript of scholastic records and diploma duly authenticated by the school registrar; G.
