1. Gain a better understanding of the process of project planning by working with a team to plan a variety of activities that will help reduce worsening incontinence in the long term care setting. 2. Learn how to develop a procedure that will be useful to the community of interest, in this case reducing worsening incontinence in long term care. Evaluation: Competencies: comment on each competency using all assignments and learning activities to support your comments i.e. participation in orientation, seminars, agency project, meetings Professional Responsibility and Accountability Within the goal of learning how to plan a project, I successfully met set out timelines by using my time management skills and organization of my workload (10). In planning this project, myself and the group ran into minor conflicts that were resolved using basic conflict resolution strategies (12). Some of these conflicts stemmed from unclear or questionable tasks given to us by our preceptors, such as feeding residents or toileting them. It was important that we questioned these orders because we need to follow our limitations as students within this placement (15, 3). This project had us integrate quality improvement principles into nursing practice (19). The goal I had for creating a procedure document has not been met because of time constraints, however we may still be able to produce some kind of document that staff can use. I have demonstrated respect for all partners involved in this project, and have positively represented Trent University. Knowledge-based Practice This placement, paired with the NURS 2000 course, has given me a wider knowledge base in the growth and development of groups, communities, and population health perspectives (26). In completing areas of this project, I understand the importance of using best practices and research to guide practice. A knowledge base in health sciences helped me to understand the consequences and causes related to incontinence (24). I have used knowledge from previous courses, particularly NURS 1002 from first year, which was about working with older populations.

Ethical Practice Throughout this semester, I have demonstrated confidentiality by shredding any documents that have residents names on them, and by not speaking about residents outside of my clinical placement (99). I have also respected the Residents Bill of Rights that is displayed and followed by all staff in the home (93). I have also respected individual residents and PSWs wishes/decisions to change a residents designated product for the reasons they disclosed, as they are the ones who are affected by this program (97). Service to the Public I got a better understanding of how an organization (Extendicare corp.) provides health care in the process of creating this project (103). In this project, it was important to establish a professional relationship with the health care team members (registered staff and PSWs) to help ensure they would implement the program as we planned, because ultimately they are the ones who will make this program successful (105c). I also participated in establishing a professional relationship with staff in order to get feedback from them about the program so that we could evaluate the effectiveness of it (106d). Self-Regulation I understand the importance of seeking new knowledge and using it in practice to ensure my competence (123d). I also understand the importance of demonstrating continuing competence by developing a learning plan, implementing it, and receiving feedback from peers and preceptors about it in order to maximize and meet my goals (123c, 123e). In some of the activities necessary for this project, it was important for me to understand and respect the parameters of my practice, for example, when measuring residents we needed to be under supervision of a staff member. Future Considerations What goals or learning activities will I be carrying into my next rotation and why? I hope to meet my unmet goal about developing a procedure in the future because it was not met in this placement. I hope to learn more about community development in the future because it was not incorporated into our project. We were assigned a project rather than asking residents and staff what their main concerns were. __________________________________________________________________________________ Artifacts chosen for enhancement of my portfolio Photo of the poster presentation Faculty evaluation (midterm and final) Preceptor evaluation (midterm and final)
