1. Describe the organizations mission, goals, programs/services.

Harden County Healthy Carolinians was formed in 1998 as one of the 5 pilot task forces
across the state. The idea was to test the model of forming a coalition in order that they
community could address its own health issues. The Task Force was originally housed at the
health department and was later moved to the Harden County Hospital where it has been housed
since that move; the task force formally became a coalition in 2000.

Harden County Healthy Carolinians had conducted numerous programs ranging from Baby
Think It Over, Cholesterol and PSA screening for seniors, and lastly, Spirit of Women heart
disease prevention program & health screenings for seniors. SAFE Kids, Body Recall in senior
exercise programs and Spit tobacco prevention in youth are all grants that Harden Healthy
Carolinians have been approved for.

















2. Provide a description of the project including key strategies, timeline for project.
The goal of A.M.Es against Heart Disease is to reduce the risk/ prevalence of heart disease in
African Americans 45 years of age and older. The program will be open to the first 25 registries
in the community that would like to participate and hopefully reduce their risk or heart disease.
This program will be a month long running from June 1, 2013 July 1, 2013. Flyers from
A.M.E.s against Heart Disease will be disbursed around the community and information will
be presented in the bulletins at church along with during the announcement part of the service,
no later than April 5, 2013. All serious inquirers will have to report to the church on May 20,
2013 in order to fill our PAR-Q forms along with informed consent forms. If signs and
symptoms are present by the information filled out on the questionnaire, the client must go see
their physician first (and be cleared) before beginning to exercise. On June 1, 2013 the 25
participants, that has been cleared to start A.M.Es against Heart Disease will begin.The
program would be specific to St. Marys A.M.E. Church. All events will be held at this location.
They will begin by getting their Blood Pressure and weight checked. This will happen at the
beginning, middle and end of the program, in order to evaluate the reduction of blood pressure
and weight change. Participants will be asked to complete a seven-day food diary each week (1
month total) in order to ensure a healthy diet. Monday, Wednesday, and Friday will be exercise
days, while Tuesday and Thursday will be learning days. MWF, would consist of a brisk
neighborhood walk, followed by Wii time. Each week the pace increases along with time (all
depending on clients). Wii time includes interactive low intensity aerobic exercises that will be
displayed for everyone to participate in. Tuesday and Thursday would consist of different
community members that are involved in nutrition to come spread some of their knowledge in
order to educate the members of their community. These days will also be healthy potluck
dinner days. On July 1, 2013, the final day of the program, the participant that has the biggest
decrease in blood pressure and waist circumference.



















3. Describe the program/need that the program will address.

Before the age of 50, African Americans heart failure rate is 20 times higher than that of
whites, according to the study the CDC conducted. Four risk factors are the strongest predictors,
and they are high blood pressure (hypertension), kidney disease, overweight and obesity and low
HDL cholesterol levels. In North Carolina alone, almost 25% of all deaths among African
Americans are a result of heart disease. The morbidity rates of African Americans versus
Caucasians are greatly varied. Amongst all deaths 309.5 African Americans died of heart
disease and stroke while 241.8 Caucasians died of heart disease and stroke between 2004 and
2008. Accompanying that, almost 22% of all African Americans in North Carolina are living
with heart disease.

-By the midpoint of A.M.E.s against Heart Disease, 50% of the participants should have a
decrease in Blood Pressure
-By the end of A.M.E.s against Heart Disease, have made changes to lifestyle (including diet).






5. List the objectives. These should be measurable.

in Blood Pressure
-By the end of A.M.E.s against Heart Disease, 100% of the participants should have a decrease
in weight

7. State how, when, and by whom an evaluation will be conducted including any reports
that will be compiled and how the evaluation will be used for program improvements.

Wake County Healthy Carolinians and St. Marys A.M.E. Church will evaluate the A.M.E.s
against Heart Disease in order to determine whether the objectives were met. The evaluations
include:

Pre, midpoint, and post data collected for Blood Pressure and weight, to evaluate the
change in both.
Number of participants who started and who finished
Evaluation of weekly food record (1month total)
Success and satisfaction of participants


After the completion of A.M.E.s against Heart Disease, a summative evaluation will be
conducted to evaluate the effectiveness of the program. The results will be given to the Perry
Foundation, St. Marys A.M.E Church and to Healthy Carolinians.

A.M.E.s against Heart Disease has became a annual program that will be hosted during the
summer months in order to reduce the risk of heart disease by reducing weight and increases
physical activity.
