I, the undersigned, am willing to participate in a research project entitled Why
do parents send their children to resource centres? carried out by Matthew Renaud, Clint Rizzo, Andrew Schembri and Thomas Farrugia. I am aware that the interview will be recorded and that any relevant information shall be used solely for academic reasons. I understand that no personal information shall be disclosed and that all recorded material shall be destroyed after completion of the project. I understand that I am allowed to withdraw from the project at any time and without having to provide a reason. I have been briefed about the nature and aims of the study, and have had the opportunity to ask further questions and seek clarifications.
