All citizen in Worth County and surround-
ing counties will be healthy, will educated,
self sufficient and residing in housing which
is clean, safe and affordable.
As the community Housing Development Organiza-
tion (CHDO) for Worth and surrounding counties,
we will partner with both public and private entities
to provide housing to very low to moderate income
residents, while providing tools for self-sufficiency.

To act as a liaison between the City, State and Fed-
eral Government Agencies, in order to provide
affordable, safe and sanitary housing, for low, very
low and moderate income residents in our commu-
nity and surrounding counties.
Family Visions Outreach donates emergency
food to needy families in Worth County and sur-
rounding areas.

Office Hours: 10am-5pm M-TH
Clients must have a State ID, proof of residency,
and fill out a brief application which is updated
annually.

Family Visions Outreach, Inc. Reentry Program is
a comprehensive and collaborative effort, which
provides services for ex-offenders and re-entrants
ages 18 and above.
Our Mission is to reduce recidivism through cul-
tivating robust, community-wide collaborative
and catalyst for systemic change that educates,
facilitates, promotes, and advocates for public
safety through effective reentry and reintegration
of formerly incarcerated individuals.
Our Vision is to eradicate the revolving door
of incarceration.
