外科手術預防性抗生素之建議使用準則

在眾多的文獻報告中指出術後傷口感染造成的死 正確的預防性抗生素施打時間應該越靠近手術處

表二、手術傷口分類（surgical wound classification）：2

第一類 / 清潔傷口 1.無發炎現象。

3.完全縫合的傷口；若有引流，則採密閉引流者。
4.非穿刺性的傷口。
第二類 / 清潔污染傷口 1.進入呼吸道、消化道、生殖道、泌尿道等管道，而無特
Class II / Clean-Contaminated Wounds 殊污染的手術傷口。
2.無感染性的膽道、闌尾、陰道、口咽的手術傷口。
3.手術過程中沒有明顯的污染。
第三類 / 污染傷口 1.開放性的、新的、意外性的傷口。
Class III / Contaminated Wounds 2.腸胃道內容物有明顯溢出、手術過程有明顯的污染者。
3.有急性發炎，但未化濃。
第四類 / 髒或己感染的傷口 1.有壞死組織的舊的外傷傷口。
Class IV / Dirty or Infected Wounds 2.內臟穿孔。
3.己有感染的傷口。
