Physical Assessment
This physical assessment is of myself. I have broken it down into sections. The first
cople sections are !estionnaires and health screenin"s to ensre that I don#t have any medical
conditions that wold be a concern for me bein" active. I also "o over an actal physical
assessment of myself in for different areas of physical fitness. After "oin" over these reslts I
discss motivational strate"ies and recommendations for stayin" fit. I also cover my readiness
for chan"e and body composition. All of the assessments were taken from Fit to Be Well
$Thy"erson% & Thy"erson% '(11).
Par-Q
The physical assessment readiness !estionnaire $Par*+) is desi"ned to screen for any pre*
e,istin" medical conditions that wold hinder yo from performin" physical activity. I answered
no to all of the !estions. I have been physically active the past cople of years bt I still looked
over the !estions to ensre none of the conditions apply to me.
Health Status Questionnaire
The health stats !estionnaire is a more in depth look at yor health stats and is
recommended before participatin" in fitness testin" $-ranks & .owley% '((/). The
!estionnaire breaks down into 0 sections to inclde% medical history% risk factor assessment%
medications and physical activity patterns and ob1ectives. I will "o over each of the sections
individally.
General Medical History
2eneral medical history is broken down into heart history% symptoms and additional health
isses. I did not have any checks ne,t to any of the !estions this time arond bt I did have a

cople of concerns a few months a"o that I did see a doctor for and therefore are worth
mentionin". They were shortness of breath and safety of e,ercise.
The shortness of breath did not occr drin" e,ercise bt drin" re"lar activity drin" the
day. I had this concern a few years a"o and bro"ht it p to my aller"ist becase I have a lot of
aller"ies. 3e eventally determined it to be 1st swellin"4inflammation in my nasal passa"es
from aller"ies. 5,ercise actally benefits this becase the adrenaline from cardio fitness cases
my nose to open p more and therefore I breathe easier.
The safety of e,ercise came abot when I started rnnin" a lot a year a"o. After abot 6
months of rnnin" I noticed my knees wold start to really bckle and be in pain from the impact
after a mile or so. They wold "et better after a day or two bt always flare p a"ain. The
doctor dia"nosed it as a common problem with rnners and it is a reslt of not stretchin" and
need for more stren"th in my le" mscles. I have responded to this with increased stretchin" if I
rn and I am takin" a Body Pmp class to help stren"then my le"s.
Risk Factor Assesment and Medications
I combined these sections becase I have no concern for risk factors or medications. The
only medications I take are aller"y medications. I have bad aller"ies to pollen so I often e,ercise
inside becase of this drin" times where it wold be nice to e,ercise otside.
Health and Lifestyle Behaviors
7y weekly e,ercise is crrently inconsistent. -or the past month I have made it to a cople
of Body Pmp classes% which are a hi"h*ener"y stren"th trainin" class. I plan on makin" them a
more re"lar part of my weekly rotine. I occasionally do a fitness dvd at home bt I am not a
fan of them. I also take a spin class once a week bt I have missed the last two for family

fnctions. I don#t like missin" these and I need4plan to make it a habit of makin" p the class if I
miss it in the ftre.
Body om!ostion
The lab assessment had me assess 8 thin"s to inclde a waist measrement% a waist to hip
ratio% my B7I% and a body pinch test. 7y waist measrement and waist to hip ratio were both
normal or nder the avera"e listed. 7y body mass inde, is abot a '8.0 so I am 1st in the
normal ran"e. I didn#t do the body pinch test becase I had access to an electronic device that
told me my body fat percenta"e and it was in the normal ran"e.
"utrition
7y ntrition I wold say is my stron"est point. 9ver two years a"o I started eatin"
healthier and that was the main reason I started droppin" wei"ht. I eliminated nhealthy fats%
watched my portion si:es% drank more water and chose healthier foods. I follow the 7yPlate
"idelines for the most part.
Readiness for han#e
7y readiness for chan"e wold is in the action sta"e. And that is for many different
aspects of my physical fitness. I wold say I take some action towards bein" active and fit% bt I
str""le with makin" them rotine. The only area where I wold feel I am in the maintenance
sta"e is ntrition.
Assessment Results$ RF
7y cardiorespiratory reslts were easily my best reslts when compared to any of my other
assessments. I scored a 0 in my mile walk with abot an 11*minte mile walk while maintainin"
a below 1/( heart rate accordin" to my heart rate monitor. 7y 1.0 mile rn $done on a treadmill)

time was in the e,cellent ratin" scale with abot 1( mintes. I received a "ood ratin" for my
step test as well.
Assessment Results$ Fle%i&ility
-le,ibility wasn#t too bad for me tho"h there is room for improvement accordin" to the
assessments. The first stretch test I scored well on with my fin"ers reachin" past my toes. The
sholder stretch was fine for me as well as I was able to clasp my hands to"ether. 7y hamstrin"%
"roin and calf stretches all needed a little work.
Assessment Results$ Stren#th
I cold not "et a hold of a dynamometer for the stren"th test. I did perform the bench press
test tho"h and I mst say I can se some work here. I scored on the low end of the "ood scale.
7y bench was abot 1;0 and I wei"h abot 1</.
Assessment Results$ 'ndurance
The final assessment done was endrance. I didn#t perform the barbell assessment becase
I lost my spotter bt I did perform both the psh p and crl p test. 7y psh*ps I was able to
"et into the e,cellent ratin" with 0( psh*ps bt I was really winded. 7y crl*ps were in the
e,cellent ran"e as I stopped when I reached '0.
Motivational Strate#ies and (echni)ues
7otivational strate"ies for me wold be tailored to my sta"e of readiness for chan"e. I
nderstand the benefits of bein" active and I do live a pretty active lifestyle. I str""le with
establishin" a rotine. This is in part de to my family lifestyle. 7y family does not have a
rotine. 7y wife works different hors based on her schedle and I am a stay at home dad with
two boys nder the a"e of 6. =reatin" a rotine for workin" ot arond a baby is difficlt bt not

impossible. >emindin" myself that it is possible to create rotine is one motivational strate"y.
The "ym I "o to has a kids "ym where they offer to watch yor kid while yo work ot.
Recommendations
7y recommendations for myself wold focs on two areas. 9ne is creatin" a rotine and
the other is focsin" on stren"th. This report and assessments have indicated that my stren"th
was the area that I needed the most focs. -le,ibility was another area that cold se some
work. 7y readiness for chan"e is in the action sta"e bt it is not in the maintenance sta"e.
Usin" this information I can work with makin" rotines and focsin" on specific areas of fitness.
The first area to address is makin" a rotine. The 2old#s "ym I have a membership to
offers rotine classes and childcare drin" those classes. 7y wife#s schedle isn#t "oin" to
chan"e whether I can "o to these classes or not. The only thin" that her schedle effects is
whether or not I take the kids to the "ym to have them watch them or leave them with her while I
"o. I wold recommend "oin" in the mornin" since the boys take a nap at 1pm and I have dinner
to take care of after they wake p. The "ym offers a body*pmp class on -ridays from ?*1(am.
This is a perfect time to "o for me. They also offer a "rop cyclin" class on @nday mornin"s
that I can attend before chrch.
-ocsin" on stren"th trainin" is another area where I need to focs my ener"ies. 9ne
problem that I have with this is I hate liftin" wei"hts by myself. I wold recommend two ways
to remedy this. 5ither findin" someone to lift with sch as a bddy or trainer. 9r I wold find
an alternate means of focsin" on stren"th. The body pmp class is a start bt there is only one
day a week that I can take advanta"e of it. The other recommend doin" fitness rotines on dvd.
This will take workin" arond my wife#s schedle becase it# hard to do a workot at home by
myself with the baby interrptin" me.

Physical Assessment
The key focal point here is rotine. I need to make sre that I plan ot my workots. I plan
on doin" the classes on -riday and @nday bt I shold try to plan for days that I miss those for
other reasons. The problem lies in that if I "ive myself a choice at any time there is a chance I
won#t work ot bt if I write it into my schedle then I am more likely to do the workot. @o
overall my recommendation wold be to e,ercise at least three days a week at the "ym and
remain physically active at home with the boys to maintain a "ood fitness level.
