Hi, Angie.
Here's the policy as it stands right now. A little warning: this is version 6 of my changes and I anticipate
more tweaking to come. However, its gone through my stakeholders twice now, so I'm hoping we're
getting really close to a consensus. I'm going to hold off on sending it back out again until after the Kaizen
meeting so that I can include any committee recommendations in this version.

Yes, please! That would be great. I'm emailing stakeholders this week with a deadline of next week, so I
should have a (mostly) approved policy to send you in time for your meeting on May 12th. Can we send it

I dont know about the screening tool. The last time I revised the policy, it did not exist. The policy has never
been reviewed by CCQI committee before. I dont know if Jana is still on the committee?

Regarding the Kaizen Committee, I would just be sure that Sylvia understands you are using her review on
behalf of the committee (I am assuming she is member?).

I saw Dr. Levin on Friday and he confirmed that he would like to review the policy.

The Screening Tool for policy revisions also indicates I should send it to someone on the Critical Care
Quality Improvement Committee; would Janna Petrie work for that?

Yes, you should send to the Medical Director of the Acute Pain Service, Matt Fiegel, MD. I think at the last
revision it was reviewed by both Jeannie Youngwerth, MD with Palliative Care and Dimitriy Levin, MD with the
Hospitalist Oncology team. I am not sure they need to review again, but you could offer to send to them if they
wish.

Rob & Barb,

As you may know, I'm working on updating the PCA Policy & Procedure with recent changes we've made at
the hospital (Alaris pumps, subcutaneous PCA infusions, and the new sedation scale). My changes are
almost totally complete--with the exception of the PCA bolus dual sign-off issue everyone's working on
right now--and I'm getting ready to send out the policy to all interested parties. Barb Wenger told me that
you're my source of truth regarding who the required stakeholders are for approval of this policy. Can you
let me know who they are?
