Child Care Contract Agreement

June 2014

I am truly blessed with the opportunity to begin this journey. I currently working towards being a licensed Family Day
care home pursuant to the State of Tennessee and Rutherford County. I have attended the pre-license orientation and
currently am working with the Department of Human Services to obtain my license. Although I do not have a license, I
am following all the requirements from the beginning so you can rest assured that your child is in the good hands.

Hours of Operation
I will be open from 6:45 until 5:45 Monday through Friday. Your child is allotted 45 hours per week. If you need other
hours, please let me know and I will try to work with you.

Drop off and Pick Up Policy
Drop off and pick up will occur at the back door. I have a porch with two doors that go to two different areas. If we
are in the main part of the home, I will have the back door open. If the front door is open, then feel free to drop
your child off at the front. I will not answer my door before 6:45am unless we have previously discussed other
arrangements. Pick up will occur no later than 5:45pm. If you see you will be running late, please call. I will allow you
to be 15 minutes late twice a month without penalty if you call me. If you do not call me, then you will be charged a
$2 per minute fee beginning at 5:45pm. This fee is due by Friday of the week or your child will not be allowed on
Monday. I will NEVER release a child to anyone including relatives unless I have talked with a parent previously.

Payment Policy
Payments are due at time of drop off every Monday. Any payment that is made after Monday at drop off will incur a
$25 a day late fee. Cash is preferred. If you write a check, I will need your drivers license number and two phone
numbers on the check. If it doesnt clear the bank, you will be charged $20 extra.

Admissions
I understand that not every facility is a good match for a child and/or family. I will accept children on a two week
trial basis. If at any time, we do not feel this is a good match, then the service will be ended upon discussion. After
the two week trial period, a two week period must be given to end service from either party. There will be a non-
refundable holding fee of $50 which will be taken off of the first week.

Daily Schedule
I am a strong believer of children having a routine. Children will have a regular schedule which will include, art, music,
play time, science, social studies, math, social skills, writing, story time nap time, meals, outside time along with home
skills and exercise. A schedule will be provided in paperwork.

Meal Times
I will provide lunch for children who can feed themselves at noon daily. I will also provide a morning snack at 9:30am
and afternoon snack at 3pm. Occasionally, I will provide breakfast for your child. I will notify you the day before if I
plan on providing breakfast the next morning. You can send breakfast with your child, but I expect that they will be
ready to start the day at 8am. Do not drop them off at 7:45 and expect us to have time to eat and clean up by
8am. This will throw our schedule off. Infants will be fed when they are hungry. I will also have prayer before meals.

Nap/Quiet Time
Nap time will be following lunch. Infants will sleep as needed. Children who do not require a nap will have quiet time.
They need to stay on their mats during this time. They will be able to look at books quietly. Nap time will be from
12:30 until 1:30.

Paid Holidays
Paid holidays are listed below. If a holiday falls on a Saturday or Sunday, I will take either Friday before or the
Monday following off. This will be posted on the schedule that I provide you every year. New Years Eve and
Christmas Eve pick up will be at 4pm unless discussed previously. Black Friday I will not open until 10am.

Vacation/Sick Days
I will have 2 weeks paid vacation every year. All families are allotted one week per year that you do not have to pay
when your child is not in my care. However, you need to inform me a month in advance to so I can make proper
arrangements for my finances. If I wake up and am sick, I will notify you immediately. I do have several people that
have had background checks done and that are my back ups if something were to happen with me. I know your work
is important and taking time off isnt easy. If I have to close due to my illness, I will adjust the weekly pay
accordingly.

Leave of Absence
If you need to request a leave of absence, please contact me to schedule a time to discuss when I do not have
children in my care. I will be happy to try to work with you.

Transportation Policy
I do not transport any children unless previously discussed with parent who leaves a proper car seat.

Pets
I do have a dog (black lab) Molly and a cat Sparky. Sparky will not be around the children at all. Molly will be crated
most of the time. If the children are ever with Molly, I will be supervising. I am currently working on getting Molly
certified to be a good citizen dog.

Smoking
Smoking and drinking any type of alcohol are not permitted at my home.

Records
I am required to keep on file certain records at all times both of myself and the children in my care. If any
information pertaining to your child needs to be updated, please inform me immediately. I am required to keep these
records for one year after care has ended. Every year I will have parents update their information. I am also required
to report any type of abuse or neglect that I might suspect.

Supplies to be provided by Families
I require families to provide the following:
One set of clothing
Sippy cup
Formula & bottles (2)
Baby food (must be sealed, no open jars will be accepted)
Diapers
Sleeping mat
Everything must have childs name on it in Sharpie
Parents are welcome to send snacks as long as there will be enough for everyone, tissues, wipes, etc. If I am in need
of any supplies I will let you know every Friday for the following week.
I will celebrate every childs birthday and every day I will pick a name and that child will be child of the day!

The most important aspect is communication between myself and parents. You can reach me any time during normal
business hours. If you need to talk to me longer than 5 minutes, please set an appointment with me. I am more than
happy to talk to you about your child and their care. I promise to communicate with you daily either at pick-up, or the
method you request. I will also send home every Friday the activities that we have done. Thank you for allowing me
