Shift report is a system of nurse-to-nurse

communication between shift changes intended


to transfer essential information for safe, holistic
care of patients. Change of shift signifies a time
of careful communication in order to promote
patient safety and best practice. There is a risk
that critical information may not be relayed
during shift report, and often the person at the
center of the communication, the patient, is not
involved. The purpose of this teaching is to
continue to implement a patient safety
strategy by consistently doing bedside
nursing shift report using an evidence-
based model.
Poor communication = Sentinel Events
Bedside Report enhances patient and nurse
satisfaction, patient involvement, better patient
outcomes, increases nurse accountability and
improves teamwork.

Why at the bedside?
Using the SBAR technique, go over high risk meds., lines and drips.
Patient and Family Involvement is aligned with our philosophy of Patient and Family
Centered Care.
1. Patient is abreast of the days plan of care
2. Care team is aware of the patients goals for
the day.
3. The patients mobility level is important for
everyone involved to know in case they
happen to be the one to assist the patient.
4. Pain management is transparent to the
patient.
5. Patient and family knows the team and the
names of the members of the care team.
Bedside Report creates a partnership
between patient, family and their care givers.
In doing leadership rounds there had been no
negative report from patients about bedside
reporting, it makes them feel involved.
The nurse gets to see their patients first thing
in the morning and will be able to answer
physician and others in the care teams
questions about the patient.
