CONSERVATION OF STRUCTURAL INTEGRITY

In order to conserve structural integrity, the
nurse encouraged to:

Provided frequent skin care.
Assess skin integrity every 4-8hrs.
Restrict sodium
Perform ROM every 4hrs.
Position patient every 2hrs.
Keep patients mouth moist and clean.

CONSERVATION OF PERSONAL INTEGRITY

In order to conserve personal integrity, the
nurse encouraged to:
Assessed changes in appearance.
Encouraged patient to verbalize
reactions and feelings about these
changes.
Assessed patients coping strategies.
Involved patient in goal-setting and
provide positive feedback for
accomplishment.
Encouraged and assist patient in
decision making about care.
Assisted patient in identifying
previous practices that may have
been harmful to self (alcohol and drug
abuse).

CONSERVATION OF SOCIAL INTEGRITY

In order to conserve social integrity, the nurse
encouraged to:
Encouraged family to support patient
towards recovery.
Provided assistance and support to
family.

CONSERVATION OF ENERGY

In order to conserve energy, the nurse
encouraged to:
Encouraged rest.
Kept patient warm.
Assisted with selection and pacing of
desired activities.
Assisted with hygiene when fatigue.
Provided oxygenation as ordered.
Assessed dietary intake.
