Regrade Request Form

All regrades must be handed in at the beginning of lecture on the class after modules are
handed back. To do so, you must fill out this form -- typed.
1. Attach this form to the front of your module with a paperclip or butterfly clip.
2. For each section you want re-evaluated, list the exercise # (e.g., 2.2A), followed
by your reasoning why you think it was graded incorrectly.
3. You must also circle the exercises on the cover page of your module.
4. You may add more sections -- if necessary.
