1. LRE: Jorge spends 40% of the time in the general education environment with moderate levels of support from a paraprofessional. Jorge will go to lunch, recess, and specials with his peers. The option of 75% in the general education environment was considered however Jorges needs in communication, behavior, and academics warrant more significant support from adults and the special education teacher. 2. Goals Math: When given the AIMS Web math computation probe (or equivalent) Jorge will recall one-digit addition facts with 90% accuracy and with no more than 3 visual prompts by 12/20/2014. Objectives: By 3/20/2014, Jorge will recall one-digit addition facts with 35% accuracy and no more than 3 verbal prompts. By 9/20/2014, Jorge will recall one-digit addition facts with 60% accuracy and no more than 4 visual prompts. Literacy: by 12/20/2014, Jorge will make appropriate text-self connections in relation to the main character using adapted 5 th grade literature with no more than one prompting question as measured by teacher made prompt-frequency tally. Objectives: By 3/20/2014, Jorge will make appropriate text-self connections using sentence starters relating to self (such as When Matilda went to the library by herself, it reminded me of a time when I __________). By 9/20/2014, Jorge will make appropriate text-self connections using adapted 5 th grade literature following teacher modeling with no more than 1 verbal prompts. Communication: By 12/20/2014, Jorge will use his choice board/PECS to communicate when frustrated in 9/10 instances with no prompting as measured by teacher made checklist. Objectives: By 3/20/2014, Jorge will use his choice board to communicate his need for a break with no more than 1 prompt. By 3/20/2014, Jorge will use his choice board to communicate his want to stop a preferred activity with no more than 1 prompt.
