FOR FORMAL FORMATIVE ASSESSMENTS:

3. Who will score the assessment, if scored?
a. The assessment will not be scored. It will be reviewed by the teacher as a check for
understanding.
4. Describe the info (data) that will be collected.
a. The students will write a ratio from given information and put it in simplest form. The
assessment will look at both of these individually. First if the students can correctly create a
ratio from given information and second if they are able to put the fraction in simplest form.
This will be shown as A and B on the assessment tally.
5. What will the teacher do with the data? How will feedback be given? Will the scores be used for
grading? If so, how?
a. The data will be used as a check for understanding. The information will be used to determine
if students have a grasp of the concept and if you need to reteach any areas. It will help to
guide further instruction. The information could also be used to create groups of students
based on their level of understanding. Future instruction can be differentiated based on
students individual level of understanding.
b. Feedback will be given either individually or as a small group based on the level of
differentiation that is needed for each student. The teacher will talk with each individual or
small group about their ticket out the door results and the information that was derived from
it, strengths and areas for improvement. The teacher will then talk about what the student(s)
will be doing to meet the areas for improvement or enrichment.
6. What is the plan for students to use the information collected?
a. The information will be used to help students understand the areas that they still need to work
on to improve their understanding of the concept. It will help them to understand what they
do well and where they still need to improve. It will help them to understand learning goals
and to know what they need to do to be successful.

8. Describe intentional, possible interventions for low scoring students (those who dont get it) and
enrichments for high scoring students (those who get it the first time).
a. This information will be used to determine areas for instruction during the next lesson.
Students will work in groups based on the results from the assessment. Students could be
placed in one of three groups: students that understood both concepts and will be doing
enrichment activities, students that will be working on creating ratios, students working on
simplifying fractions, and students that need to work on both areas. For students that need
more help they will use manipulatives and instructional videos to increase understanding.
Students that understood the concepts will work on enrichment activities. They will be given

more difficult ratios to construct and simplify as well as problems in story problem format.
This will encourage students to think about the information that is necessary and not
necessary and to correctly construct the ratios based on the given information. As students
are working the teacher will rotate through the groups to check for understanding and provide
additional instruction and assistance.
