Goals
1) To learn at least one new thing about my assigned resident each time I attend my
clinical placement until the end of the semester.
2) To perform hand hygiene as necessary (or whenever in doubt) when practicing in
the clinical setting until the end of the semester.

Activities, Necessary Resources and Timeframes to achieve my goal


Activities:
Genuine conversation between myself and the client/patient (e.g. regularly
asking things like, How has your week been?, What types of activities
do you enjoy? or How is your family?)
Ensure to actively listen to everything that my resident has to say.
Confidentially document what I learned about my resident and feedback
received in my personal journal to help me see the progress I made in my
relational practice skills over the course of one semester.
Using hand sanitizer or soap and water whenever deemed necessary (or in
doubt) while in the clinical setting
Going over reminders as to when hand hygiene should be performed
before every day attend clinical.
Documenting in personal journal when the instances for hand hygiene are
reviewed.

Feedback from my resident, peers, other staff at the facility, and clinical
instructor.

Timeframe: I will conduct the activities listed above each time I attend clinical practice
until the end of the semester.

Evaluation
Did I achieve my goal and what is the evidence?

1) I will know that I have achieved my goal when I look through my journal at the
end of the semester and read about the relationships I built with my resident
through the course of my clinical placement. I will note whether or not I did learn
one new thing each day about him/her, and what my peers, resident, facility staff,
and clinical instructor said about my relational practice skills. From there I will
determine what I did right, or wrong, and then either translate my actions from my
1020 placement into future placements, or make a plan on how to improve my
relational practice skills so that this goal can be attained in the future.
2) I will know that I have achieved my goal when I review my personal journal to
note whether or not I did review the instances for which hand hygiene should be
performed regularly, and by evaluating feedback I receive from my colleagues,
my assigned resident, facility staff, and clinical instructor with regards to my
proficiency in maintaining competent hand hygiene. From there I will determine
whether or not my plan to achieve this goal is effective and what amendments
should be made, or what practices should be maintained.

Goals
1) To incorporate and promote confidentiality practices into my clinical practice
every day that I attend clinical placement and whenever I notice that clinical
practice (specifically its residents) become a topic of discussion outside of the
clinical setting through till the end of the semester.
2) To be considerate and aware of any unique cultural or spiritual beliefs my
assigned resident may practice throughout the entire semester.

Activities, Necessary Resources and Timeframes to achieve my goal


Activities:
Implementing confidentiality practices such as informed consent for daily
decisions that are to be made for the health of my assigned resident
Closing doors whenever personal information is being disclosed/discussed
between either my assigned resident and myself or colleagues and their
assigned residents
Using names such as Patient X or Colleague X or initials when
making notes in my personal journal, and requesting feedback from
colleagues, other facility staff, my assigned resident.
Not make assumptions about his/her cultural or religious/spiritual
affiliations and practices
Advocating for my assigned resident whenever health care practices
interfere with cultural or spiritual beliefs important to the resident
Providing alternatives, and accommodating (whenever possible) to my
assigned resident whenever particular health care practices may interfere
with his/her cultural or spiritual beliefs
Document in personal journal anytime cultural or spiritual circumstances
require any sort of alteration in care including reflection on the

Necessary Resources:

My assigned residents chart on which any significant cultural or spiritual
practices would be indicated
External resources (e.g. Internet, RNAO BPGs) when more knowledge
about a certain culture or religion, or providing care for individuals with
certain cultural or religious beliefs would be beneficial in providing my
assigned resident with more culturally sensitive care
Timeframe: I will conduct the activities listed above each time I attend clinical practice
and anytime confidentiality of residents at the long term care facility I am assigned to
becomes an area of concern, until the end of the semester.

Evaluation
How will I know that I have achieved my goal?

1) I will know that I have achieved my goal when I evaluate the notes I took in my
personal journal and any feedback I receive from colleagues, other facility staff,
my assigned resident, or clinical instructor. When evaluating these, I will be
comparing my own personal notes on any events involving confidentiality
practices I took action with to the feedback I receive from the various sources
listed above. I will note whether my view on my level of competence in
maintaining/promoting confidentiality is lower, equal to, or better than the
feedback I received. This will tell me if and what changes should be made, and/or
what went right that I should implement in future clinical settings to ensure I can
accomplish this goal with every placement I enter.
2) I will know that I have achieved my goal when I review my personal journal to
see if I faced any instances where skills of cultural sensitivity, or awareness were
required to maintain quality, patient-centered care, what actions I took in these
situations, and how these experiences affected me (reflections). I will note what
went well/wrong in these situations so that I may improve upon/maintain my
cultural/spiritual sensitivity and awareness skills, and apply them into future
clinical settings.

Goals
1) To ensure that I do not work outside my scope of practice throughout the semester
at my long term care NURS 1020 clinical placement.
2) To maintain an adequate level of personal fitness to better perform certain health
care practices (e.g. lifts and transfers) throughout the rest of the semester.

Activities, Necessary Resources and Timeframes to achieve my goal


Activities:
Writing up a document every week that outlines my scope of practice
Looking over this document before I start each shift at my clinical
placement
Updating my resident every week what new skills I have learned and are
then able to apply and reminding them that I cant perform certain
requested tasks if they are outside of my scope of practice
Establish back up support with a staff member at the long term care
facility I am assigned to ensure I have a more knowledgeable health care
provider that can assist me when my resident or another staff member
asks me to perform a task that may be outside of my scope of practice.
Keep a personal diary to help me confidentially reflect on my days at my
clinical placement, in which I will write about any situation in which my
limited scope of practice became an inconvenience or caused me to seek
these external supports.
Exercising a minimum 3 days a week (1-2 hours each)
Cardio, strength, and flexibility exercises)
Creating a schedule/checklist to use as a guide for ensuring weekly
workouts are completed
Checking off weekly workouts as they are completed

Documenting in personal journal as performing clinical skills requiring an


adequate level of personal fitness become either easier or harder

Timeframe: The activities listed above are ones that I will take action with each time I
attend clinical practice until the end of the semester in April 2013.

Evaluation
How will I know that I have achieved my goal?

1) I will know that I have achieved my goal when I discuss my professional conduct
surrounding my scope of practice during the NURS 1020 clinical placement with
those that I have established supportive relationships with (facility staff and
clinical instructor) and receive either positive or negative feedback, and when I
look back at my personal clinical diary and establish whether or not my scope of
practice was breached at any point during my attendance at clinical practice.
2) I will know that I have achieved my goal when I look over my workout schedule
and determine whether or not I did follow my schedule, and when I look over my
personal journal and note whether or not performing clinical skills requiring an
adequate level of personal fitness either become easier or harder. This will tell me
if my plan to achieve this goal requires amendments or is effective as is (in which
it will be maintained for the rest of my career).

Goals
1) To incorporate aspects of the Excellent Care For All Act (ECFAA) into my
clinical practice until the end of the semester.
2) To keep myself up to date on all major nursing and health care issues occurring
regionally, provincially, and nationally throughout the remainder of the semester.

Activities, Necessary Resources and Timeframes to achieve my goal


Activities:
Asking permission of the resident for any and all health care actions to be
taken
Documenting the residents decisions in his/her medical record
Reviewing the ECFAA periodically (every one-two weeks) to ensure
requirements around patient-centered care are followed
Requesting feedback from colleagues, my assigned resident, facility staff,
and clinical instructor on my consistency on placing my assigned resident
at the centre of his/her health care.
Keeping track of and reading various nursing/health care journals and
peer-reviewed nursing/health care articles on a regular basis
Documenting in personal journal when I do read a nursing journal or peerreviewed article related to health care, what I learned from it, and what I
can apply into the clinical setting.

1) I will know that I have achieved this goal when I evaluate the feedback I receive
from colleagues, my assigned resident, facility staff, and clinical instructor and

note whether or not I was consistent in requesting permission for my residents


health care.
2) I will know that I have achieved my goal when I note whether or not I did read
nursing/health care journals and peer-reviewed articles on a regular basis in my
personal journal, and if I noted what was learned/can be applied in the clinical
setting as these journals and articles become available to me. This will tell me
whether or not I am keeping myself up to date on current nursing and health care
issues regionally, provincially, and nationally, and therefore whether or not
changes to how I reach this goal should be made.

1) To participate in the CNOs Quality Assurance (QA) program throughout my
experience in clinical practice until the end of the semester.
2) To ensure that I always have a clean, wrinkle free pair of scrubs to wear to
clinical practice throughout the remainder of the semester.

and clinical instructor about my progress as I learn and apply new clinical
skills.
Laying out a clean pair of scrubs every night before clinical to ensure they
are free of wrinkles
Setting up a laundry schedule to ensure scrubs get washed
Regularly washing my scrubs, and only wearing them once, to ensure a
clean pair is always accessible
Necessary Resources:
Personal clinical diary, and
Feedback from colleagues, my assigned resident, facility staff, and clinical
instructor.
Necessary laundry utilities (e.g. washer and dryer, detergent and dryer
sheets etc.)

Timeframe: The activities listed above will be performed throughout my attendance at
clinical placement and planned reflective time taken place after attending clinical
placement until the end of the second semester in April 2013.

Evaluation
How will I know that I have achieved my goal?

1) I will know that I have achieved my goal when I make note of whether or not I
documented feedback I received, and wrote about my clinical experiences during
planned reflective times. I will then evaluate my reflections and documented
feedback and note what went well/wrong over the course of my 1020 clinical
placement, and therefore what about my practice I should continue into future
placements or change in order to continue progressing as a nursing student.
2) I will know that I have achieved my goal when I review my laundry schedule
(and closet) to determine whether or not I have followed my laundry schedule and
have clean, wrinkle free scrubs to wear. I will also evaluate feedback I receive
from colleagues, my assigned resident, facility staff, and clinical instructor with
regards to my consistency in wearing only clean and wrinkle free scrubs to
clinical practice. From there I will determine whether or not my plan to achieve
this goal is effective and what amendments should be made, or what practices
should be maintained.
