{need some assistance with a couple of issues pertaining to the 2 subject providers/physicians:
1. Dr. Cassidy —I still need/require some information as it relates to Dr. Cassidy’s duties and
responsibilities, Dr. Cassidy's percent level of effort (LOE) changed from 100% to 20% as of 1/6/2009. As
a result, Ronnie Smith had stipulated back in late January/early February that we would need to process and
route a new Letter of Offer (LOO) for him delineating his new percent LOE, new title, new remuneration
and new duties and responsibilities related to his new position/status with LSUHSC and the LSU SOM. 1
know all of the pieces here with the exception of what it is we expect from him in the way of services
provided given his new, diminished LOE with the organization. could make up some semblance of what 1
think his duties could be, but, in this case, given his status as a U.S. Congressman, I think it would be
prudent from our perspective if | knew exactly what it is ya'll expect from him for his one day per week
(20%). Is he to provide simple clinic services, GI services, endoscopies, HIV services, etc.? What happens
to his clinical trials? Is he to provide services at the private clinic? 1 wouldn't think so given that I can’t
reimburse him for those services anyway since those collections/payments would put him over the $25,000
threshold and auditors are going to want to know exactly what itis he is doing for us (EKLMC; LSUHSC)
for the $20,000 Base pay we give him annually, We are going to really have to spell out exactly what it is
he does for us for his remuneration fiom us. Believe me, this scenario will be a very auditable item and 1
feel they will really hone in on this situation to make sure we are meeting all federal and state

per — Iwas recently told by @asilillgmst that you have him going to Hunt Correctional
effective March, 2009 3 Friday afternoons per month, Well, with these new additional duties/clinics I need
to process a new LOO for him changing his percent LOE from 90% to 100%, What sort of compensation
(besides the 10% increase in base pay) did you negotiate with him in the way of additional supplement? $
per clinic? $ per month? Please advise as soon as you can so that we may move forward with his new
LOO. Thanks,
