Weekly Evaluation

1. This week I was able to implement nursing care using leadership, communication, and
decision making as part of an interdisciplinary team by collaborating with my nurse on
our patient who was on the neuro unit, working with the PAs and doctors to figure out if
it was best for him get a transfer from the ICU onto a neuro floor. I did a neurological
assessment on this patient and was able to use those assessment findings and collaborate
with the patients team to make a decision on transferring him. The patient ended up
getting a transfer, so I think the input I gave had some effect on that.
2. The type of patient care I gave with my patient this week was care I dont think I have
ever felt so appreciated by. He lost sight a month ago and called an ambulance, after
multiple tests, a tumor was found on his pituitary gland. He was on a ventilator and
highly sedated when I got into his room. I tried talking to him and squeezing his hand to
get him to wake up, after turning his sedation off, he became a lot more alert. I went up to
him after I could tell he was uncomfortable. I held his hand and talked to him, although
he couldnt talk back, I asked him questions and had him answer with squeezing my hand
once for no and twice for yes. I could tell he was very uncomfortable so I tried walking
away to tell the nurse, but he wouldnt let go of my hand. Every time I tried letting go he
would squeeze harder. I asked him if he wanted me to just stand with him and hold his
hand and he squeezed my hand twice. It was hard to see a patient like this and no family
there to comfort him. Although I was giving much nursing skill care to him at this time,
I felt like what I was doing meant so much to him. I feel like that was an environment
that made him feel safe because I knew he was scared.
3. With the blind patient that I had, we weaned him off of his sedation to see if he was able
to tolerate the ventilator because the doctor wanted to try and take him off of it. Being
with him so much, I could see how irritated he was, so after a couple hours and working
with the respiratory therapist, I was able to tell my nurse right away about how he was
and get him back on the sedation so that he was uncomfortable.
4. Being on this floor it is extremely important to make sure the patient is getting the most
safe and quality of care because of how sick of every patient as how hard it is on the
families that are involved. Being exposed to such serious situations I am able to act non
judgmental (especially with the patient two weeks ago that grabbed the wheel of his
wifes car while intoxicated) I gave him the best care I could even though it was hard
knowing the history of why he was there. I was very respectful toward his family and
also I was able to collaborate with the medical team while making decision for his care.
