! Describe the districts vision of a check-in/check-out process for
9th graders.
! Establish group norms to have more productive meetings
! Discuss our vision for how check-in/check-out would at Lincoln
East
Consequences:
! Decided on how the process will flow at Lincoln East.
! Discussed how next time it would be nice to have more
information from the middle schools
Behaviors:
! I distributed information about the expectations from the district
! We held a brainstorming session on ideas for Lincoln East
! We had a long discussion about group norms where everyone got
to express their thoughts and feelings.

! Resources Ensure staff has the necessary resources. I did this
by describing and distributing district information.

I did this through my brainstorming sessions.

! Designed and edited forms used to describe, and detail the
different aspects of SPARTAN Success (check-in/check-out)
! Discussed how we were going to identify students for SPARTAN

! Finalized the process for how SPARTAN Success will look at
Lincoln East
Consequences:
! We will create a 15-day check for all teachers to fill out to help
us identify students for SPARTAN Success
! We will draft a brief email describing the intervention to the staff
! Next year I would like more input from people who were
involved with the intervention this year

depending on the audience

! Communication Develops effective ways for staff to
communicate. I did this with the development of our forms for
the staff members not on the PBiS Tier 2 team.
! Change Agent Consciously change the status quo. I did this
with the 15-day check. It was a new for staff members at East to
have to record and thing about their struggling students and how
they have supported them.

! Reviewed 15-day check master list to identify 9th graders for
SPARTAN Success
Consequences:
! Next we will talk with Students, Parents, and identified Case
Workers about the intervention
! Next year I will have staff nominated students for SPARTAN
Success because we will be more familiar with the process

! We reviewed the 15-day check to identify 9th graders who were
struggling with grades, referrals, and attendance.
! We identified if the students we had concerns with should start
on SPARTAN Success or be in the monitor stage

! Focus Continually keeps attention on established goal. Our
goal for the meeting was to identify students who are struggling
and get them started on SPARTAN Success.

potential Case Managers
! Contacted Case Managers to describe their role in the SPARTAN
Success process

! Next meeting we will review the data on the students involved

! Next we will talk with the district level PBiS Tier 2 trainers to
figure out what other schools are doing
! Next week I will meet with Synergy people to discuss possible
grading reports

! Discussed what the program should look like and what other
schools were doing with their check-in/check-out
implementation.
Consequences:
! Next we will develop a plan to meet with the staff and to reteach
them about the purpose of SPARTAN Success.
! Next week we will analyze students who are on the SMART
team report.

! Discussed what was working and what we could learn from other
schools.
! Talked about our student indicators and if they should be
adjusted.

what was working well and what needed to be improved in their
check-in/check-out process.

! We will develop a lesson detailing what students would be good
candidates for SPARTAN Success.

current situation. We were struggling to identify students for
SPARTAN Success. We decided to work with LSW to help
develop a plan for getting more staff input.
