82 years old, she had a diagnosis of inflammatory bowel
disease and had no bowel movement for 5 days. She was
ordered Dulocolax 10mg; suppository once a day prn.
After thoroughly assessing my client, I provided a nursing
diagnosis of constipation related to inability to eliminate as
evidenced by no bowel movement for 5 days. I continued
on to provide a client centered outcome and nursing
interventions, along with re-assessing my client for
abdominal distension.
What I learned from this course is that identifying the

I have learned that the nursing diagnosis provides a
selection of nursing interventions to achieve outcomes,
which I; the nurse will be accountable for. I have learned
that the focus is on the client outcomes and use of the
SMART criteria to achieve the best for the client. This is
meaningful because it will allow me to critically think and
perform assessments, resulting in me being safe, confident,
and competent in my practice as a Practical Nurse.

related to inability to eliminate. I then made a client
centered outcome related to the nursing diagnosis, that the
client will have bowel movement within 2 hours of
administering Dulocolax. Before administering the
medication I performed the Ten Rights of Medication,
assessed the client, and administered the suppository
medication. I reassessed the patient after administration of
the medication to ensure that there was no unpredicted side
effects/adverse effects, and ensured that the medication is
achieving its therapeutic effect. I reassessed the client and
taught the client to increase their fluid intake to at least
