RISK ASSESSMENT METHODOLOGY

The followings need to be considered prior to and while conducting the risk assessment:
1. Area of survey ranging from well sites, flowlines, processings, pipelines, storages
2. Define Location
3. Define the context.
It could be Activities, Equipment or Facilities.
4. Looking for the Potentials Hazard or HSE issues.
5. Assess the risk with the following Risk Assessment Guideline
6. Identify risk control measures in place.
7. Provide recommendations (if any) to mitigate the risk with the principle of
ALARP (As Low As Reasonably Practicable).
8. Provide Estimated Cost for following up the recommendations
9. Provide estimated time frame for completion recommendations.
10. Provide Department or personnel in-charge for completing the recommendations.
11. Fill in the form attached.
12. Provide report on regular basis to the management of the status.
Risk Assessment Guideline
The risk assessment is conducted by a Team consists of personnel from various
departments. The Team assesses risks of potential hazards that appear during the survey.
Risk ranking is made based on a matrix below. (The matrix was developed by DNV
Technica).
