Community Agency Project

BSMCON- NUR3111P
Purpose: As future nurses, it is imperative for nursing students to be aware of and able to refer
clients to the myriad of community health agencies in our area. As such, this activity provides an
avenue for students to research a particular community-based agency and understand the services
that the agency provides.
Instructions: To become familiar with the assigned agency and the services provided by it, the
student must research the assigned community agency by accessing internet resources, visiting
the agency briefly, and obtaining printed resources about the agency, if available.
The following questions should be used to guide the students research about the agency. The
student will provide a brief presentation to classmates, using the attached grade rubric as a guide.
The student will also provide a hand-out about the agency to be used for future reference by
classmates.

Have blood pressure higher than 120/80


Have blood sugar levels over 100
Have cholesterol levels over 200
Not have private health insurance
Have no insurance provided by your employer
Not qualify for government assistance
Earn less than 200% of the Federal Poverty Guideline
7. What services are provided by agency?

We relieve the financial burden on the community by providing care to people who
have chronic conditions, preventing inevitable and costly outcomes.
We provide prescription drugs for a nominal fee to people who need medications but
could not otherwise afford them.
We teach people with chronic health problems how to manage their lifestyles to
improve overall wellness. This means they miss fewer days from work and employers reduce
health-related productivity losses.
We relieve the burden on full-service clinics by serving a targeted population with
appropriate and efficient care.
We reduce costly visits to hospital emergency rooms by people who are uninsured.
We provide specialized care and free study medications for qualified individuals
involved in our medically supervised research studies.
