Lauren's confidence and ability to articulate

the role of Occupational therapy in this
setting has improved significantly. She has
demonstrated an ability to accurately
articulate this to families and colleagues.

Lauren has only had the opportunity to

evaluate 2 children and observe 3
evaluations due to low need for evaluations
after count ending on Nov. 1st. She will
have more opportunity in the coming weeks.
During the evaluations she has completed
she demonstrated good clinical judgement
and completed her evaluation write ups with
thoroughness and was very concise.
Lauren has proven herself to be flexible and

competent in administering, scoring,
documenting, and reviewing the results of
the PDMS-2 with the therapy team and
families.

Lauren has worked with and collaborated

with COTA's but has not yet had the
opportunity to assign responsibilities. This
will be further addressed in the next 6
weeks.

Lauren has demonstrated great

communication skills with the other
therapists and with the classroom teachers

she is working with. She communicates well
with families however she will benefit from
increasing her confidence in talking with
families about recommendations and
rationale for treatment sessions.
Lauren effectively communicates with the

children to increase their understanding of
tasks presented to them. She is able to
articulate to families the rationale for
evaluation tasks and treatment plans, and
communicates with team members
professionally.
Comments on strengths and areas of improvement:

Lauren has gone to activities such as

language work shops outside of business
hours and is very receptive to constructive
criticism.
Lauren has high expectations for herself

and has demonstrated this consistently
throughout her 12 weeks here at CDC.

Lauren has been competently establishing

goals and treatment plans for our entire
caseload for the last 4 weeks of her
fieldwork.
