AFFIDAVIT OF WITNESS

I, Luis Manzano, legal age, single, resident of Ayala Heights, Quezon City,
after having sworn in accordance with law, do hereby depose and state THAT:

I am the owner and operator of Angels Taxi with Main Office

located at 272 Quezon Avenue, Quezon City;

I certified that one of Angels Taxi unit is the White Toyota taxi cab

with plate number WWW-123;

3.

The said White Toyota Taxi Cab with plate number WWW-123 is

driven by one of our Taxi Drivers, Phil Younghusband as per records in our
Main Office;
4.

That requirements for qualifications as a taxi driver are as follows:

a. Applicant must be from 20-55 years of age,

b. Possesses a professional drivers license as certified by the Land
Transportation Office.
c. Able-bodied with no history of any heart ailment, epilepsy or other
ailments of the like,
d. Natural 20/20 vision.

5. That I certified that Wicked Witch is not connected in any way as

driver, maintainance, staff or operator of Angels Taxi.
