AFFIDAVIT OF LOSS

2. I lost said I.D Card on February 26, 2015 and I could not
remember exactly where I placed/left my wallet containing the
said I.D Card and it is considered lost for all legal purposes;
3. That all efforts have already been exerted to find it but
only proved futile;
4. I am executing this affidavit to attest to the foregoing
facts and for whatever lawful purposes it may serve.
