outcome is defined to be met through the engagement and collaboration with the interprofessional health care team. In order show how I have achieved this outcome within my years at Bon Secours College of Nursing, I have attached three documents above. The first document is from my first semester junior year, NUR 3112P - Adult Nursing Science II Practicum: Adult Nursing Science II Practicum allows students to continue to build the knowledge, skills, and attitudes regarding the essential holistic care of adults across the healthcare continuum. Students have the opportunity to provide nursing care to clients with common acute and chronic health problems in primary, secondary, and tertiary settings. This practicum is closely aligned with Adult Nursing Science II and Pathopharmacology II. The second link is from my second semester junior year NUR 3114P - Mental Health and Mental Illness Nursing Practicum assignment: The Mental Health and Mental Illness Nursing Practicum focuses on the practice of therapeutic communication and evidence-based practice in the care of clients with mental health and mental illness concerns at primary, secondary, and tertiary preventative levels of care. Emphasis is placed on the holistic application of biopsychosocial, behavioral, and communication theories. The nursing process guides nursing care, with emphasis on a collaborative, interprofessional, and clientcentered approach. Students will perform
psychosocial assessments, lead therapeutic
groups, provide mental health education, and provide direct care to mentally ill clients. Students will participate and reflect on a service learning activity. The third link is from my last semester as a senior, through NUR 4143 Clinical Immersion where I was assigned to write Scholarly Reflective Journal at Midterm: Reflective journals will document professional growth through the students practicum experiences. I chose to include these specific documents as highlighted the progressive instances where I have worked with community organizations, healthcare staff, and/or peers to help meet the common goal of helping others attain good health. Through my experience in the Caravan I was able to become part of a team of social workers, translators, medical assistants, nurses, and physicians whom help those that are uninsured. Through my experience at the YMCA, I was able to work with the local YMCA Healthy Start Program workers to build a child appropriate lesson on resilience that would build a solid foundation on handling future mental health stress. Lastly, through my final reflection in Immersion, I was able to show how communicated well with the healthcare team on the Medical Surgical Unit to identify and aide a patient in health crisis.
