Over the course of the semester, I


believe that I have demonstrated safe
and competent practice as I have
accepted responsibility for my own
actions whether it be signing off for a
medication when administering it to a
patient or documenting accurately
regarding a patients condition. I have
also demonstrated professionalism as I
have never been late or absent for
clinical. I have also dressed
appropriately for clinical and asked
relevant questions pertaining to a
patients condition. I have also
continuously incorporated the
knowledge I have gained from nursing
classes in clinical practice.

I have maintained a therapeutic and


caring relationship with my clients as I
have attended to their needs and applied
the skills I have learnt through the
previous semesters in order to provide
appropriate and accurate assessments.
If they were experiencing any pain I
would assess what would cause them
pain and would carry out the appropriate
actions. If the patient were to refuse any
form of care I would assess why and
respect their decision. When providing
care, communicating with my teacher or
fellow students, I have never acted
aggressively or rudely towards them.
Whenever a nurse or fellow student
needs help I am always there to help the
individual- changing of briefs and
helping with various assessments such
as vital signs. If another student has
done something wrong I try to help them
fix it and explain to them what they have
done wrong and how they can change it
in the future in order to provide the best
possible care to their clients.

have continued to help my peers
whenever help is needed whether it be a
question regarding a patients condition
or helping with repositioning a patient. I
have been an advocate for my patients
as I have continued to provide care and
assess the patient in order to determine
if any additional measures need to be
put in place to promote a therapeutic
environment. Also, in order to promote
my understanding of the patient I ask
questions and ask for help regarding
their condition. If I am doing something
wrong I am able to take the constructive

Throughout the semester I believe that I


have meet the clients needs by
performing head-to-toe assessments in
order to get a better understanding of
the patients needs. I have continuously
incorporated previous assessment
techniques learnt in other semesters to
enhance my techniques. Whenever the
patient complains about pain I use the
PQRSTU model to indentify and
understand what the patient is
experiencing. Whenever meeting and
leaving the patients room I perform
appropriate hand hygiene. When I
assess the patient I ensure that they
have good hygiene, if they need to be
changed or washed I ensure that it has
been completed. I also promote the
patients safety by lowering the bed
when I leave their room and also putting
the side rails up. If they patient requires
restraints I make sure consent is
obtained and they are applied
appropriately. Continuously, in order to
ensure I have a good understanding of
the patients condition I fill out the
weekly step 1 tool. In order to provide
the best care for the individual I perform
various assessments (cardiac,
integumentary, neuro, respiratory etc.)
to determine if something needs to be
reported to the nurses or doctor. Also in
order to get a better understanding of
the clients condition I ask them various
questions throughout my assessments
which contribute to better care for the
patient. Also, when obtaining report
from the nurse I always check the code
status of my patient to understand what
needs to be done when an emergency
occurs and in order to respect the
wishes of the client. Whenever entering
an isolation room I make sure that I
correctly put on PPE and ensure that all
visitors of the patient understand how to
wear PPE and why they should wear it.

Throughout my clinical practice I have


maintained quality assurance by
following a learning plan that I have
created and now I have developed an
additional one in order to improve on my
weaknesses. This plan has guided me in
determining my strength and
weaknesses and the new one I created
will also help me improve on my
weaknesses. This learning plan has
allowed me to focus on my difficulties
and try to improve them while practicing
in the clinical setting. I have also used
peer and teacher feedback to improve
my current practice in order to be
successful. I have also applied the
theory I have learnt throughout the
course of the nursing program in the
clinical setting, and whenever a fellow
peer or nurse is doing something which
I have never done or seen I always ask if
I am able to observe what they will be
doing in order to further my practice.
