Chris has jumped in this semester and easily transition into the facility and is
extremely efficient and at easy with residents meeting them at their level. I'm pleased
to have him!

This student has served his first semester at another facility and has only been at our
facility since early January, less than 7 weeks. Therefore, there hasn't been the
opportunity to present some of the competencies because of the timeframe. I have no
doubt that the semester will afford the opportunity for Chris to address and succeed in
all competency areas.

the students performance, as indicated by the rating that is most frequent of the

indicate that they have discussed the evaluation, even if the student does not agree

Comments:
LP from first term not available. Chris is highly motivated, easily adjusting to the high
demands of the facility in a very short time. He appropriately uses supervision and
seeks information to be the best intern he can be. I am impressed with Chris' ability to
come to a facility in the middle of his year (having lost his internship at the previous
location) and be able to blend well with other established interns at this facility and
the social services team. His attention to detail and interaction with the residents at all
levels is impressive. We are glad to have him at Warren Woods.

I did get a lot of practice but it was only available for a limited time, but I agree that I
didn't have a lot of opportunity for things
