postal address at 49 Masbate St., Batasan Hills, Quezon City, After having been duly sworn to in
accordance with law, do hereby depose and say:
1. That my source of income is derived from my capacity as FARMER;
2. That from this activity, I am deriving monthly income of TWENTY-ONE THOUSAND
PESOS (Php. 10,100.00) or more;
3. That with said income generate enables me to meet my needs and of my family;
4. That of the same income, I am also enabling to meet my other obligations I may Enter into
the near future.
5. That this Affidavit is executed as part of my documentary requirement in the National
Housing Authority, processing of my papers and for whatever legal intents purposes this may
serve be the best.
