Abstract

In this critical analysis report I will describe an organization within the Porter State Service
Center, where I am currently completing my internship. I will discuss the program, give detail to
who is responsible for running the program, how its managed and if I was running the program;
what I would do differently.

The program that I will be describing at the Porter State Service Center is their Division
of Public Health Clinic. There is a clinic inside the Porter State Service Center that delivers
sexual and reproductive health services in an urban setting that includes related disease
interventions investigations for those in need of these services. They provide individuals and
families with health and wellness services such as testing for breast, cervical, prostate, and
colorectal cancer to eligible individuals by providing Office visits, Mammograms and clinical
breast exams for breast cancer, Pap tests, Prostate cancer screening tests recommended by your
doctor, Colorectal cancer screening tests recommended by your doctor, Health education, Help
with coordinating associated care. They also provide dental services, immunizations, WIC and a
lot more services.
They also provide newborn screening that consists of newborn metabolic disorder
screenings, hearing screening, birth defects and autism surveillance programs. These screenings
intended to identify newborn babies with one of a number of rare disorders. Babies with these
disorders may appear well at birth. Most of these disorders, if not identified and treated soon
after birth, can result in developmental delay, substantial cognitive disability, serious medical
problems, or even death (www.dhss.delaware.gov). There are some disorders that may be
detected that have no proven definitively effective treatment. Newborn screening, together with
rapid confirmatory diagnosis and treatment prevents cognitive delay, illness, and death in
newborns. It also saves millions of dollars in treatment, home, and institutional cost
(www.dhss.delaware.gov).
The clinic also provides people with HIV testing along with other sexually transmitted
disease testings. They also provide tuberculosis screenings. These clinics are staffed by Public

Health Nurses (PHNs) who specialize in tuberculosis, and by Pulmonary Consultant Physicians.
The clinics provide direct medical care and follow-up to patients with latent TB infection and TB
disease. All screening activities, contact investigations and case management services are
provided through the clinics (www.dhss.delaware.gov). This clinic provides a multitude of
services for those in need.
The program is run by a Clinic Manager who is responsible for managing clinic staff in
the delivery of sexual and reproductive health services in an urban setting that includes related
disease intervention investigations. Their duties include: making decisions affecting the daily
clinic operations including personnel utilization, clinic scheduling, quality assurance, monitoring
clinic activities and evaluating cost effectiveness of service delivery, ensuring proper clinic staff
coverage of all services and adherence to service standards and policies, and conducting client,
clinic, community and staff needs assessments. The program manager is responsible for ensuring
the all daily operations runs smoothly and efficiently and that all clients needs are getting met in
a professional timely manner.
Currently from what I have been seeing at the Porter State Service Center is that the
program has been running poorly due the lack of staff and the frequent absence of the current
clinic manager and staff. You cant run an effective organization if you are never there. There are
a lot of staff call outs which ultimately results in delayed waiting times for the clients as there
isnt enough staff to service them in a timely manner. With staff shortages, clients arent
receiving the adequate care that they need and deserve. Also the staff that are there are feeling
overworked and overwhelmed which explains the high ratio of call outs. This program is
supported by state and federal funding.

If I was the director of this program I would adhere to all policies and procedures of the
organization and also implement new policies and procedures accordingly to the needs to staff
and the organization. I would talk with the current staff to analyze issues that have led to failure
in the past and get a better understanding of their issues and concerns and make necessary
changes to make my staff feel more appreciated and would want to come to work instead of
constantly calling out. Also I would immediately look at the budget to see if I would have the
ability to hire more staff so that we can properly accommodate the needs and services of the
consumers in a timely and professional manner. I would focus on the strengths of the
organization such as the program is there to provide needs and services to individuals with little
to no income and being in the heart of Wilmington there are a lot of people that rely on her
services.
The weakness at the clinic is the lack of staff and the high ratio of staff call outs we
cannot provide the necessary services. The strengths of the clinic is that we are located in the city
and our clinic is easily accessible to everyone that needs to come as we are directly on the bus
line so people dont have any issues with finding our location. Even when some peoples primary
reason isnt to come to our clinic but for other reasons we can encourage them to come because;
we are in a building with other needed services we can often educate people on the services we
provide so they can come back for services. Although the clinic receives federal and state
funding, I would reach out to respective donors so there will never be an issue for funding for the
clinic. I would pass out brochures, hold rallies along with other events to raise awareness of the
clinic and funding.
As a director the potential staff that I would be looking for is someone who is passionate
about helping others. Someone who is dedicated, determined, passionate, has great customer

service skills and has the ability to work hard and under pressure. Someone that is fresh with new
ideas that can be implemented to make the program a lot better. Having the ability to adapt to
any situation, and generate new ideas would be a wonderful component to our team. Our goal is
to provide the best services possible to our consumers to make them want to return again. To me
these are key factors of and employee because; when you enjoy the work that you are doing and
are very passionate about it customers can see that and will feel welcomed to return for services.
The same would apply for an intern, I would look for the same qualities in an employee as I
would in an intern. The passion and the love for what you are doing is key.
In this paper I have described a program separate from the program that I am currently in
as an intern with at the Porter State Service Center. I have detailed the program and what they do.
I have also described their issues currently and what I would do if I was a director at this
establish and what I would do differently which is to hire new staff, address the issues and
concerns of the current staff, implement new policies and procedures, hold events to secure more
funding and find employees that are passionate about the work that they do.
