Action Plan for Module # 5 Action # 2

Goal: 1. To develop and offer an asthma management education class to anyone in the community prior to August 2014, the members will be
graded on their average test scores with a goal of 75% or higher.
2. Nationally everyday 36,000 kids miss school because of asthma. With the implementation of our course our goal is to decrease the
national number by decreasing our districts asthma related absences by 10%.
Results/Accomplishments: Having an asthma management course available for anyone that attends or lives in our school district.

Assessments of student knowledge. Our benchmarks will be all students passing the course with a 75% or
