1.

Describe your position. For each of the major responsibilities, what were the expectations and
outcomes?

To demonstrate leadership through the assessment, planning, implementation and


evaluation of critical care patients in the eICU according to the standards of professional
nursing practice. Nursing care is provided to the individual patient through remote
monitoring with a goal of early detection of warning signs of clinical deterioration, based
upon cultural, spiritual and age specific needs. Rapid assessment and interventions by
the eRN leads to fewer mortalities and complications in the ICU patients. The eICU
Center registered nurse (RN) is a member of a collaborative care team who is
responsible for providing tele-nursing care to a diverse population of critically ill adults
across multiple ICUs at various hospitals. The following features govern the role of the
eICU Center staff RN: Patient advocacy, Proactive & Continuous Monitoring,
Prevention, Evidence-based care, Telecommunication, Clinical Decision Support,
Humane & Compassionate Ethical Care, and Patient Privacy.

eRNs are required to attend staff meetings, administrative activities, provide


departmental in-services, contribute to performance improvement projects, continuing
education efforts, supervise delegation to unlicensed personnel, communicate with
other members of the healthcare team effectively, utilize technology effectively in patient
care initiatives, advance patient plan of care, prioritize patient care issues, and
demonstrates aspects of financial stewardship through effective time management and
resource allotment.

Describe any projects you have been responsible for which are not in your position description.

Sole nightshift preceptor and clinical advisor for nightshift. Neither of these projects are
in my position description. I have oriented every new nightshift employee myself for the
last three years. The role of clinical advisor is an uncompensated role similar to charge
nurse and is an informal leadership role, according to the department manager. She
seeks advise from me as to needs and workflow on the nightshift in her absence and
the nightshift staff can come to me as a spokesperson for their needs.

Describe any changes you suggested and/or implemented this year that led to improvement in your
area (e.g., quality of work life, cost savings, efficiency).

Scheduling changes were adopted, communication changes, workflow changes for


efficiency, and increased staffing with responsibility for said staff were taken into
consideration.

What action(s) have you taken this year to gain a better understanding of the organization, your
unit/division/department, or your own job?

Attend meetings, review job description regularly, confer with management as needed,
communicate with team members, continuing education, and sought opportunities to
network with telecommunication expert for 1:1 education.

5.

Give examples of departmental collaborative efforts that you were involved in this year. What
contributions did you make to the team?

Skilled nursing facility rapid response initiative: 1:1 education on the nightshift with
follow up debriefing with SNF staff to educate to the process. Allows for familiarity with
process and ease of use with technology to better care for patients.
Unit Liaison: 1:1 liaison with ICU contact meeting via face to face and phone meetings
to facilitate cohesion in unit collaborations and patient care. Communicated results with
team members who are also liaisons.

What goals have you set for yourself during the next year? What types of projects would you like to
be involved in?

Utilize my education degree in a more active role within the department - perhaps in the
form of continuing education initiatives or projects for the department and expanding
from there.

What types of developmental activities would you like to take advantage of (e.g., seminars, specific
training classes, etc.)?

I would like to participate in telecommunication seminars, Users Group, SCCM, and


NTI.

8.

What kind of support and/or guidance would you like to see from your supervisor?

I would like to see more support of continuing education from my direct supervisor, to
include those initiative that require financial aid incentives.
9.

What are your strengths/weaknesses?

I am an effective two-way communicator with a passion for nursing and education. I


lead by example and promote learning through maintenance of excellence. I can be
flexible as the need arises and learn from every unique situation as they present
themselves. Through my various roles in healthcare and leadership, I will be able to
offer learners culturally diverse experiences using real-world examples and the latest
technologic approaches within the changing face of healthcare. Being a novice
educator, I will continue to build my skill in curriculum development and educational
offering delivery as mastery will continue over years to come with active practice and
reflection. Feedback from peers and learners will be sought in order to improve.
Organizational participation, certification in education, and dissemination of knowledge
through publication are other areas for further improvement in the future.
