APPLICATION FORM FOR ADMISSION INTO DIPLOMA AND CERTIFICATE

PROGRAMMES FOR THE ACADEMIC YEAR 2015/2016
SECTION A: Instructions to the applicants
a) Complete the application form using capital letters
b) Application forms must be accompanied with a copy of Bank pay-in slip of
application fee Tsh.20,000/= non refundable. List of agricultural training institutes
and their respective bank accounts is attached for use and reference.
c) All application forms must be duly filled, signed, attached with certified copies of
birth certificate, academic certificates and transcripts.
d) Applicants must meet all entrance requirements of the programme she/he is
applying for, otherwise, applications shall not be processed.
e) Employed applicants should attach release letter from their employers
f) Applicants should post the application form direct to the institute(s) she/he is
requesting for admission not later than 30th June 2015. List of agricultural
training institutes and their respective postal address is attached for use and
reference.
g) Successful applicants shall be informed through their postal address or E-mail
address and Ministrys Website: www.kilimo.go.tz

h) Qualifications
1)

Certificate Course
Certificate in General Agriculture (NTA level 5): applicant must have at
least four (4) passes of grade D in either Physics, Chemistry, Biology,
Geography, Mathematics and Agriculture in the Ordinary Certificate of
Secondary Education. English will be an added advantage.

2) Diploma Courses
i. Diploma in Land Use Planning: applicant must have Advanced Certificate
of Secondary Education with at least two (2) subsidiary passes in either
Physics, Chemistry, Biology, Geography, Mathematics and Agriculture.
Mathematics is compulsory. OR Applicant must have passed Certificate in
General Agriculture (NTA level 5) and must have passed at least four (4)
subject in either Physics, Chemistry, Biology, Geography, Mathematics and
Agriculture with grade D in the Ordinary Certificate of Secondary
Education.
ii. Diploma in Irrigation: applicant must have Advanced Certificate of
Secondary Education with at least two (2) subsidiary passes in either
Physics, Chemistry, Biology, Geography, Mathematics and Economics.
Mathematics is compulsory. OR Applicant must have passed Certificate in
General Agriculture (NTA level 5) and must have passed at least four (4)
subject in either Physics, Chemistry, Biology, Geography, Mathematics and
Agriculture) with grade D in the Ordinary Certificate of Secondary
Education.
iii. Diploma in General Agriculture : Applicant must have passed Certificate
in General Agriculture (NTA level 5) and must have passed at least four (4)
subjects in either Physics, Chemistry, Biology, Geography, Mathematics and
Agriculture with grade D in the Ordinary Certificate of Secondary
Education. OR applicant must have Advanced Certificate of Secondary

Education with at least two (2) subsidiary passes in either Physics,
Chemistry, Biology, Geography and Agriculture.
iv. Diploma in Food Production and Nutrition: applicant must have
Advanced Certificate of Secondary Education with at least two (2) subsidiary
passes in either Physics, Chemistry, Biology, Geography, Nutrition and
Agriculture. OR Applicant must have passed Certificate in General
Agriculture (NTA level 5) and must have passed at least four (4) subjects in
either Physics, Chemistry, Biology, Geography, Mathematics and Agriculture
with grade D in the Ordinary Certificate of Secondary Education.
v. Diploma in Agro mechanization: applicant must have Advanced
Certificate of Secondary Education with at least two (2) subsidiary passes in
either

Physics,

Chemistry,

Geography,

Mathematics,

Economics

and

Agriculture. Mathematics is compulsory. OR Applicant must have passed

Certificate in General Agriculture (NTA level 5) and must have passed at
least four (4) subjects in either Physics, Chemistry, Biology, Geography,
Mathematics and Agriculture with grade D in the Ordinary Certificate of
Secondary Education.
vi. Diploma in Horticulture: applicant must have Advanced Certificate of
Secondary Education with at least two (2) subsidiary passes in either
Physics, Chemistry, Biology, Geography, Nutrition, and Agriculture. OR
Applicant must have passed Certificate in General Agriculture (NTA level 5)
and must have passed at least four (4) subjects in either Physics, Chemistry,
Biology, Geography, Mathematics and Agriculture with grade D in the
Ordinary Certificate of Secondary Education.
vii. Diploma in Crop Production: applicant must have Advanced Certificate of
Secondary Education with at least two (2) subsidiary passes in either
Physics, Chemistry, Biology, Geography, and Agriculture. OR Applicant must
have passed Certificate in General Agriculture (NTA level 5) and must have
passed at least four (4) subjects in either Physics, Chemistry, Biology,

Geography, Mathematics and Agriculture with grade D in the Ordinary
Certificate of Secondary Education.
