DIRECTIONS:
Fill out The Planning Document; select two goals that will impact student growth and evidence to support them from the data menu.
These goals will be connected to the Frameworks For Teaching. The educator will discuss this plan with their administrator for approval.
(October 1 Deadline for PGP completion with administrator)

The educator will gather evidence, demonstrating progress of the above, mentioned goals and present the evidence and data along with
the results to the administrator at the Summary Meeting. (June 1 Deadline for PGP completion with administrator)

Please answer these questions prior to the Goal Setting Meeting. Your responses will then serve as the basis for your conversation during
the Goal Setting Meeting.

1.

What are (at least) two goals you will set for yourself this year? (Connect to the Frameworks)

Domain 1c: My goal is to use learning targets with fidelity and reference them at the beginning, middle, and end of each lesson. I also
plan to use learning targets that are: clear, accessible, and ensure that students provide evidence of their learning
Domain 2e: My goal for this domain will focus on the physical space in my classroom.This will largely include the layout of our room
and how that is decided. I will focus on students leading our room layout. I will also support our learning through physical space by
providing organized areas where relevant anchor charts are placed from lessons.
Domain 3c: Use district & state goals of increasing proficiency of math on the district common assessment. My goal is to move
towards 85% of my students reaching proficiency by the end of the year assessment.
Domain 4b: I plan to focus on conferring/conferencing with students during Reader's Workshop. I will keep clear and accurate records
to reflect my conferring with fidelity during this time

What sources are you using to gather data to support your goals? (Use data menu)
The sources I plan to use are end of the year inventory tests and form A math tests.
I also plan to use phtots, conference notes, activote surveys, interviews, and goal-setting data.

Clearly summarize what the data tells you about your students growth.
My data will tell me if students in my class are performing at a proficient level in Math. My data will tell me if I am using learning
targets and conferring effectively and with fidelity. My data will also tell me if I organize my physical space to the benefit of my
students and their learning environment.
4.
What implications does this data have for your goals this year?
The data will display the level of academic student success in reading and math as well as my professional level of success during
conferring, using learning targets, and planning physical space in our room.
5.
How do these goals fit in with the district improvement plans, school improvement plan and/or the content standards?
These goals align with the district initiatives put in place over the past year as well as our one page strategic plan. They also align
with my schools improvement goals, and the district initiatives regarding annual evaluation development and professional goals.

What evidence will you collect this year to determine if you have met your goals?

I will collect the end of the year inventory tests, or unit test forms A. I will also collect my conferring notes and record student growth
data in reading based on DRA2 and Dibels scores. I will use interviews from students to determine fidelity with learning targets and
conferring.

How will you use the evidence and data collected to guide your planning throughout the school year?
As the year progresses, I will continue to monitor student progress towards success in these areas. If I find students are not making
progress I will use intervention adjustments to help meet these goals.

Domain 1: Planning and Preparation
1f: To assess student learning by checking for understanding at the end of each lesson.
1f: To both formally and informally assess student learning on a daily basis and check for understanding.
1f: To be more aware of where my students stand in terms of their understanding on the homework assignments.
1f: My goal is to develop and implement coherent, logical, and thorough assessments for each unit I teach in my
classes. I will implement multiple forms of assessment, including essay tests, fill-in-the-blanks, multiple choice,
and verbal response. Homework will also be created to act as assessments of learning and understanding of
content.

Domain 2: The Classroom Environment


2a: To create an environment free from bullying, harrassment and negative language.
2a. Make sure the students are conscience of how their actions affect others.
2b: To implement the Capturing Kids Heart curriculum within my classroom.
2b: Take an interest in students besides just through math curriculum.
2c: To make good use of the new technology in my classroom (smart cart) and feel comfortable doing so.
2e: Organize my physical space to encourage and enable learning, and to facilitate group discussions. I also want
to organize the space to enable individuals to work alone, and to gather as groups quickly, when asked or needed.
I also want my room to be understandable for the kids, in terms of where to find materials, etc.

Domain 3: Instruction
3a: To promote personal responsibility in students by giving them a monthly calendar of planned activities and
hold them responsible for proper gear and readiness.
3b: My goal is to continue to develop my ability to engage my students in meaningful, purposeful discussions
about the curriculum. I want to develop new discussion techniques and implement them successfully. I want to
spend a small portion of each hour (most days) discussing homework, tests, or papers.
3d: To give better, more complete constructive and instructive feedback to students on their larger projects both
before and after the project is due.

Domain 4: Professional Responsibilities


4a: To write down reflections of teaching after each unit to improve the flow of the unit for next year or next
trimester.
4c: My goal is to communicate more frequently with families, for reasons concerning attendance and grades. I
will call for as many positive reasons as I will for reasons that might not be considered "positive."
4e: To develop after school technology training for colleagues to further understand and implement the new
technologies available.
