Purpose:

To create a script for your improvement effort and support implementation.

Directions:

1. Use this form to develop an action plan for each Action identified through the SHI Module assessment process.

Evidence Of Success (How will you know that you are making progress? What are your benchmarks?)

We will see a decrease in our rates of bullying and discrimination incidents within our school district at all age
levels. We would like to see a decrease of no less than 8 percent of incidents in the first year followed by a decrease
of 2 percent each year after that.
Evaluation Process (How will you determine that your goal has been reached? What are your measures?)
Our goal will be met once we have reduced the rate of bullying and discrimination incidents in our school district by 75 percent. This will be measured
through analyzing yearly behavior reports as well as the exit survey taken by the graduating seniors.
